摘 要:腹膜刺激征中腹壁紧张度(AWT)增高是腹膜炎诊断乃至是否手术的依据,但因多种原因腹膜刺激征常存在不确定性,是导致严重创伤时腹部仍然是诊断“最后的黑箱”的重要原因。借助现代科技已开发出量化AWT的仪器和方法,文章介绍了腹壁解剖结构、腹腔功能与AWT的关系,阐述了腹腔压力容积曲线、腹部顺应性及影响AWT的主要因素,重点介绍了基于LaPlace定律的AWT测量方法及其初步临床应用研究。The increased tension of the abdominal wall in the peritoneal irritation is the basis for the diagnosis of peritonitis and for the decision on whether an operation should be performed.However,there are often uncertainties in the peritoneal irritation,which lead to the fact that abdomen is still the"last black box"in the diagnosis of severe trauma.With the help of modern technology,instruments and methods for measuring abdominal wall tension(AWT)have been developed.This review introduces the relationship between abdominal wall anatomy,abdominal cavity function and AWT,expounds the abdominal pressure-volume curve,abdominal compliance,and the main factors affecting AWT,and introduces the clinical application and research progress of the AWT measurement method based on LaPlace's law.
