摘　　要：目的探讨二维斑点追踪技术(2D-STI)在评价左室不同质量指数的原发性高血压患者右心室功能变化的价值。方法选择86例高血压病患者,分为正常左室质量指数(LVMI)43例,高左室质量指数组43例;健康对照组42例,每个患者均行二维超声心动图(2DE)获得常规参数,然后应用2D-STI获得右室游离壁整体纵向应变(RVGLS),比较各组参数是否存在差异。结果高LVMI组的RVGLS绝对值比正常LVMI和健康对照组均减小,正常LVMI组的RVGLS绝对值也较健康对照组减小,差异有统计学意义(P<0.05)。高LVMI组的右室面积变化率(FAC)比健康对照组减小,差异有统计学意义(P<0.05),正常LVMI组的FAC与健康对照组和高LVMI组差异均没有统计学意义(P>0.05)。高LVMI组的心肌功能指数(Tei指数)比健康对照组和正常LVMI组均增大,差异有统计学意义(P<0.05),正常LVMI组的Tei指数与健康对照组差异没有统计学意义(P>0.05)。结论原发性高血压患者右室功能发生损害,高LVMI组患者的右心室功能发生损害更为严重。2D-STI可早期发现右室功能的变化,为临床诊治提供重要参考。Objective To investigate the value of 2D speckle tracking imaging(2D-STI)in evaluating right ventricular function in essential hypertension patients with different left ventricular mass indexes.Methods 86 patients with hypertension were divided into the normal LVMI(left ventricular mass index)group(43 cases),the high LVMI group(43 cases).And there were 42 cases in the healthy control group.Each patient underwent 2D echocardiography(2DE)to obtain routine parameters.Then 2D-STI was used to obtain the right ventricular global longitudinal strain(RVGLS),and the parameters of each group were compared.Results The absolute value of RVGLS in the high LVMI group was lower than that in the normal LVMI group as well as that in the healthy control group.And the absolute value of RVGLS in the normal LVMI group was also lower than that of the healthy control group.The above differences were statistically significant(P<0.05).The FAC of the high LVMI group was lower than that of the healthy control group,and the difference was statistically significant(P<0.05).The FAC of the normal LVMI group was not significantly different from that of the healthy control group or the high LVMI group(P>0.05).The myocardial function index(Tei index)of the high LVMI group was higher than that of the healthy control group and that of the normal LVMI group,indicating statistically significant differences(P<0.05).There was no significant difference in Tei index between the normal LVMI group and the healthy control group(P>0.05).Conclusion Patients with essential hypertension have impaired right ventricular function,and patients with high LVMI have more serious impaired right ventricular function.2D-STI can detect the change of right ventricular function in early stage,which provides an important reference for clinical diagnosis and treatment.
