骨科住院总医师心理健康现状及影响因素研究

摘　　要：目的了解骨科住院总医师培养阶段的抑郁、焦虑、睡眠障碍等状况并分析其心理障碍的影响因素。方法以北京大学第三医院骨科的住院总医师培养阶段的23名医师为研究对象,分别采用抑郁筛查量表、焦虑问卷和睡眠状况自评量表调查其抑郁、焦虑和睡眠障碍状态,并分析其影响因素。结果23名住院总医师心理不健康检出率82.6%,78.3%有抑郁症状(18/23),73.9%有焦虑症状(17/23),100%有睡眠障碍(23/23)。担任院总一年以上的人群的抑郁量表(P=0.043)、焦虑量表(P=0.021)和睡眠状况量表(P=0.045)的得分明显高于担任院总一年以内和结束院总工作的人群。“安排患者入院”“安排患者出院”和“跟上级医师沟通”是最能给住院总医师带来心理压力的三种临床因素。结论骨科住院总医师培养阶段抑郁、焦虑、睡眠障碍的比率较高,学校和医院应该深入了解骨科住院总医师培养阶段的心理状态和问题,加强心理健康教育和心理疏导,进一步完善住院医师培养管理制度。Objective To understand the depression,anxiety,sleep disorders and other conditions of the orthopedic chief resident training stage and analyze the influencing factors of their psychological disorders.Methods Taking 23 chief residents in the resident general physician training period of the Peking University Third Hospital as the research object,the depression screening scale,anxiety questionnaire and sleep self-assessment scale were used to investigate their depression,anxiety and sleep disorders,and analyze its influencing factors.Results The detection rate of 23 chief residents for mental health was 82.6%,78.3%had depressive symptoms(18/23),73.9%had anxiety symptoms(17/23),and 100%had sleep disorders(23/23).The scores of depression scale(P=0.043),anxiety scale(P=0.021)and sleep status scale(P=0.045)of people who served in the hospital for more than one year were significantly higher than those within one year The crowd who finished the general work of the hospital."Arrange patient admission","Arrange patient discharge"and"Communicate with superior doctor"are the three clinical factors that can bring the most psychological pressure to the chief resident.Conclusion The incidence of depression,anxiety,and sleep disorders in the training period of the orthopedic chief resident is high.Schools and hospitals should have a deep understanding of the psychological state and problems of the orthopedic chief resident training stage,strengthen mental health education and psychological counseling,and further improve the chief resident training management system.
