摘 要:目的探讨智能尿布法尿常规检测在新生儿急性肾损伤(AKI)早期诊断中的应用价值。方法选择2018年4月至2020年5月因各种病因入住医院新生儿重症监护室的300例新生儿作为研究对象,入院后12 h内采集静脉血3 ml,进行血肌酐检测,另采集尿液5 ml,采用智能尿布法和尿液干化学法进行尿常规检测,分析两种方法检测结果的一致性,并以血肌酐检测结果为金标准,分析智能尿布法尿常规检测对新生儿AKI的诊断效能。结果两种方法检测亚硝酸盐、尿胆原、尿蛋白等尿常规九项指标的Kappa值均>0.6,平均一致率为96.39%。血肌酐检测结果显示,300例新生儿中,56例为AKI;智能尿布法尿常规检测中,尿蛋白和潜血两项指标的诊断效能最优;尿蛋白、潜血联合检测诊断新生儿AKI的灵敏度、特异度、准确度、阳性预测值及阴性预测值均在80%左右,均高于两者单项检测。结论智能尿布法尿常规检测在新生儿AKI早期诊断中具有较高的应用价值,该方法具有便捷、智能等优点,可解决婴幼儿留尿困难、尿液易被污染、时机难以把握等诸多难题。Objective The application value of detection of urine routine by intelligent diaper method in early diagnosis of neonatal acute kidney injury(AKI)was explored.Methods 300 newborns admitted to the neonatal intensive care unit of the hospital due to various causes from April 2018 to May 2020 were selected as the research objects,3 ml venous blood was collected for blood creatinine detection and 5 ml urine was collected for routine urine detection by intelligent diaper method and urine dry-chemistry method within 12 hours after admission.The consistency between the intelligent diaper method and the urine dry-chemistry method in the detection of various indicators of urine routine was analyzed and the diagnostic efficacy of intelligent diaper method for neonatal AKI was analyzed by taking the diagnostic results of serum creatinine as the most accurate standard.Results The Kappa values of nine urinary routine indexes such as nitrite,urobilinogen and urinary protein detected by urine dry-chemistry method and intelligent diaper method were higher than 0.6,and the average consistency rate was 96.39%.The serum creatinine results showed that among 300 newborns,56 were diagnosed with AKI,while the rest were not.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the urine routine indexes detected by intelligent diaper in the diagnosis of neonatal AKI were high,in which the diagnostic efficiency of urinary protein and occult blood was higher.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of combined detection of urinary protein and occult blood in the diagnosis of neonatal AKI were about 80%,which were higher than those of single detection of urinary protein and occult blood.Conclusions The detection of urine routine by intelligent diaper method has high application value in the early diagnosis of neonatal AKI.This method has the advantages of convenience and intelligence.It can solve many problems such as difficult retention of urine,e
