摘 要:目的利用超声弹性成像分析不同年龄健康男性睾丸杨氏模量(YM)值的范围并探索其内在的变化规律。方法选取2019年1月至2020年5月保定市第一中心医院收治的248名常规睾丸检查正常的志愿者作为研究对象。按照年龄将其分为青年组(n=107)、中年组(n=66)及老年组(n=75)。应用实时剪切波弹性成像(SWE)技术对睾丸内部的不同区域进行YM值测量并分析。结果睾丸内部不同测量区域YM值比较,差异具有统计学意义(P<0.05);表现为:前包膜下(6.08±0.61)kPa=后包膜下(6.13±0.82)kPa>上包膜下(4.06±0.56)kPa=下包膜下(4.03±0.52)kPa>中央区(3.09±0.32)kPa。YM值与年龄呈正相关(P<0.05)。不同区域YM值和年龄的线性回归方程为:中央区:YM=2.512+0.012×年龄,前包膜下:YM=4.868+0.025×年龄,后包膜下:YM=4.527+0.033×年龄,上包膜下:YM=2.853+0.025×年龄,下包膜下:YM=3.053+0.020×年龄。结论SWE技术能够量化睾丸硬度,临床在参考睾丸YM值数据评估病情时,应当结合患者年龄因素及睾丸内部探查区域进行综合分析。Objective To analyze the range of Young's modulus(YM)value of testis in healthy men of different ages by ultrasound elastography and explore its rule of changes.Methods 248 volunteers with normal routine testicular examination admitted to Baoding First Central Hospital from January 2019 to May 2020 were selected as the study subjects.They were divided by age into young people(n=107),middle-aged people(n=66)and elderly people(n=75).Real-time shear wave elastography(SWE)was used to measure and analyze the YM values in different areas of the testis.Results There are significant differences in YM values in different measurement areas inside the testis(P<0.05),which expressed as:under the anterior capsule(6.08±0.61)kPa=under the posterior capsule(6.13±0.82)kPa>under the upper capsule(4.06±0.56)kPa=under the lower capsule(4.03±0.52)kPa>central area(3.09±0.32)kPa.The YM value was positively correlated with age(P<0.05).The linear regression equation of YM value and age in different regions was:central area:YM=2.512+0.012×age,under the anterior capsule:YM=4.868+0.025×age,under the posterior capsule:YM=4.527+0.033×age,under the upper capsule:YM=2.853+0.025×age,under the lower:YM=3.053+0.020×age.Conclusions SWE technology can achieve quantification of testicular hardness.Clinical evaluation of the condition with reference to the YM value should be combined with a comprehensive analysis of factors such as age and the inner testicular area.
