阴茎海绵体破裂早期行修补术后的临床疗效观察

摘　　要：目的初步评价阴茎海绵体破裂患者早期行修补手术的术后临床疗效。方法选取2017年1月至2021年1月首都医科大学附属北京朝阳医院泌尿外科诊治的10例因阴茎海绵体破裂并早期行修补手术患者的临床资料进行回顾性分析。收集患者受伤原因、受伤到手术的时间、手术方式、住院时间、并发症等指标。术后进行随访,记录患者勃起功能及性活动的恢复时间,观察有无长期并发症。结果所有患者均行急诊手术治疗。患者年龄范围19~68岁,中位年龄41.5岁,平均年龄41.1岁。其中1例因手淫时过度弯曲阴茎致伤,2例因阴茎勃起时不慎摔倒致伤,余7例均因粗暴的性交动作致伤。患者从受伤至就诊用时2~24h,平均10.6h。手术时间30~180min,平均用时93.9min。术中出血量10~50mL,平均出血量22mL。患者住院时间2~10d,平均3.9d。术后随访时间12~48个月,平均28.8个月。患者于术后1~12个月恢复性活动,平均3.8个月。10例患者均无阴茎硬结、勃起疼痛或勃起功能障碍,无排尿异常,其中2例患者出现轻微的阴茎弯曲畸形。结论阴茎海绵体破裂患者病史及临床表现典型,诊断明确后早期行急诊手术治疗是确切有效的治疗方法,有利于患者的术后性功能恢复,减少术后并发症。Objective To evaluate the clinical effect of early surgical treatment for patients with rupture of corpus cavernosum. Methods The clinical data of 10 patients with rupture of the corpora cavernosa who underwentearly emergency repairing operation in the Department of Urology of Beijing Chaoyang Hospital,Capital Medical Universityfrom January 2017 to January 2021 were retrospectively analyzed. The injury causes,time of visiting hospital,operation methods,hospital stay,complications were collected. Postoperative follow-up was performed to recordthe recovery time of erectile function and sexual activity,and observe whether there were long-term complications.Results All patients underwent emergency surgery. The patients ranged in age from 19 to 68 years,with a medianage of 41. 5 years and a mean age of 41. 1 years. Among them,1 case was caused by excessive curvature of the penisduring masturbation,2 cases were caused by inadvertent fall during erection,and the remaining 7 cases werecaused by coarse sexual intercourse. The time from injury to treatment was 2-24 hours,with an average of 10. 6hours. The operation time ranged from 30 to 180min,with an average time of 93. 9min. Intraoperative blood losswas 10-50mL,with an average of 22mL. Patients were hospitalized for 2 to 10 days with a mean of 3. 9 days. Thepostoperative follow-up time was 12 to 48 months,with an average of 28. 8 months. The patients regained sexual activity1 to 12 months after surgery,with an average of 3. 8 months. None had penile induration,erectile pain,erectiledysfunction or micturition abnormalities,and 2 of them had minor penile curvature. Conclusions The patientwith rupture of the corpora cavernosa had a typical history and clinical presentation. Early emergency surgery afterdiagnosis is an exact and effective treatment,which is beneficial to the postoperative sexual function recovery and reducepostoperative complications.
