摘　　要：目的探讨铲状电极经尿道前列腺等离子剜除术(PKEP)对良性前列腺增生(BPH)患者术后恢复及性功能的影响。方法选取2017年1月至2020年2月徐州市贾汪区人民医院诊治的102例BPH患者进行前瞻性研究。采用随机数字表法将患者分为A组(n=51)和B组(n=51)。A组采取铲状电极PKEP治疗,B组采取经尿道前列腺等离子切除术治疗。比较两组围术期相关指标,术后随访1~6个月,统计两组术前、术后3个月、术后6个月男性性健康问卷射精功能障碍部分简表调查问卷(MSHQ-EJD)、国际勃起功能评分问卷-5(IIEF-5)评分。结果A组住院时间、膀胱冲洗时间、导管留置时间较B组短,血红蛋白丢失量较B组少,差异具有统计学意义(P<0.05);术后3个月、术后6个月,两组MSHQ-EJD、IIEF-5评分均较术前降低,但A组高于B组,差异具有统计学意义(P<0.05)。结论应用铲状电极PKEP治疗BPH,可降低手术创伤,促进术后恢复,减轻对性功能的不良影响。Objective To investigate the effect of spade electrode transurethral plasmakinetic enucleation of prostate(PKEP)on postoperative recovery and sexual function of patients with benign prostatic hyperplasia(BPH).Methods A prospective study was conducted in 102 patients with BPH admitted to Xuzhou Jiawang District People's Hospital from January 2017 to February 2020.The patients were divided into group A(n=51)and group B(n=51)by random number table method.Group A was treated with spade electrode PKEP,and group B was treated with transurethral plasma prostatectomy.The perioperative indicators of the two groups were compared,and the postoperative follow-up was 1 to 6 months.The male sexual health questionnaire ejaculation dysfunction questionnaire(MSHQ-EJD)and the International Index Erectile Function(IIEF-5)of the two groups were analyzed before surgery,3 months and 6 months after surgery.Results The hospital stay,bladder irrigation time and catheter indwelling time in group A were shorter than those in group B,and the hemoglobin loss in group A was less than that in group B,with statistical significance(P<0.05).At 3 months and 6 months after surgery,MSHQ-EJD and IIEF-5 scores in 2 groups were lower than those before surgery,but group A was higher than group B,with statistical significance(P<0.05).Conclusions The treatment of BPH with spade electrode PKEP can reduce surgical trauma,promote postoperative recovery and reduce the adverse effects on sexual function.
