摘  要:目的研究血清转化生长因子β_(1)(TGF-β_(1))、单核细胞趋化蛋白-1(MCP-1)、低氧诱导因子-1α(HIF-1α)水平与慢性肾衰竭(CRF)患者肠道菌群的相关性。方法纳入2015年1月~2019年1月我院收治的111例CRF患者为CRF组,将同期于我院体检的健康者50例作为对照组,收集其一般资料(性别、年龄、原发疾病)、临床资料[血尿氮素(BUN)、血清肌肝(SCr)、血红蛋白(Hb)、估算的肾小球滤过率(eGFR)]、及血清TGF-β_(1)、MCP-1、HIF-1α水平并比较。采用Pearson相关分析探讨CRF患者血清TGF-β_(1)、MCP-1、HIF-1α与实验室指标及不同菌群的相关性。采用线性差别分析(LDA)分析菌种的差异。结果CRF组患者BUN、SCr、TGF-β1、MCP-1、HIF-1α水平与对照组相比均显著上升,eGFR及Hb水平均显著下降(P<0.05)。Pearson相关分析结果显示,CRF患者血清TGF-β1与SCr呈正相关,与eGFR呈负相关(P<0.05);血清MCP-1与BUN、SCr均呈正相关,与eGFR呈负相关(P<0.05);血清HIF-1α与BUN、SCr均呈正相关,与eGFR及Hb均呈负相关(P<0.05)。血清TGF-β_(1)、MCP-1、HIF-1α与毛螺菌科、大肠埃希菌属及肠杆菌科水平均呈正相关,血清TGF-β_(1)与克雷伯菌属水平呈正相关,血清TGF-β_(1)、MCP-1与疣微菌科水平均呈负相关,血清TGF-β1与双歧杆菌及乳酸杆菌水平均呈负相关,血清HIF-1α与双歧杆菌水平呈负相关(P<0.05)。结论CRF患者存在肠道菌群失衡,其条件致病菌丰度较健康者增加,且与血清TGF-β_(1)、MCP-1、HIF-1α水平相关,提示肠道菌群失衡可能加重CRF患者炎症反应与肾纤维化,参与CRF进程。Objective To investigate the correlation of serum transforming growth factorβ1(TGF-β1),monocyte chemoattractant protein-1(MCP-1)and hypoxia inducible factor-1α(HIF-1α)levels with intestinal flora in patients with chronic renal failure(CRF).Methods A total of 111 CRF patients admitted to our hospital from January 2015 to January 2019 were included in the CRF group,and 50 healthy subjects who underwent physical examination in our hospital during the same period were selected as the control group.General data(Gender,age,primary disease),clinical data[Blood urea nitrogen(BUN),serum creatinecscr),hemoglobin(Hb),estimated glomerular filtration rate(eGFR)]and serum TGF-β1,MCP-1,HIF-1αlevels were recorded and compared between the two groups.Pearson correlation analysis was used to analyze the correlation between serum TGF-β1,MCP-1,HIF-1α,laboratory indexes and different bacterial flora in CRF patients.Linear discriminant analysis(LDA)was used to analyze the species differences of intestinal microbiota.Results The levels of BUN,SCr,TGF-β1,MCP-1 and HIF-1αin CRF group were significantly higher than those in control group,and the levels of eGFR and Hb were significantly lower than those in control group(P<0.05).Pearson correlation analysis showed that serum TGF-β1 was positively correlated with SCr,and negatively correlated with eGFR(P<0.05);Serum MCP-1 was positively correlated with BUN and SCr,and negatively correlated with eGFR(P<0.05);Serum HIF-1αwas positively correlated with BUN and SCr,and negatively correlated with eGFR and Hb.Serum TGF-β1 and MCP-1 were positively correlated with Lachnospiraceae,Escherichia coli and Enterbacteriaceae levels,and serum TGF-β1 was positively correlated with Klebsiella level;Serum TGF-β1 and MCP-1 were negatively correlated with Verrucomicrobiaceae level,and serum TGF-β1 was negatively correlated with Bifidobacterium and Lactobacillus level;Serum HIF-1αwas negatively correlated with Bifidobacterium level(P<0.05).Conclusion The imbalance of intestinal flora of RF patien
