摘 要:目的探讨颅脑超声检查联合血清检测神经元特异性烯醇化酶(NSE)、S100β、白细胞介素-6(IL-6)在早产儿脑白质损伤诊断中的联合应用价值。方法选取有脑白质损伤的早产儿和无脑白质损伤的早产儿各60例,分别作为研究组和对照组,均行血清NSE、S100β、IL-6检测及颅脑超声检查。对照检查结果,并分析四项检查联用对早产儿脑白质损伤的诊断效能。结果研究组的早产儿出生后24 h、72 h和1周的脑白质损伤检出率分别为90.00%、86.67%和85.00%;对照组出生后24 h、72 h和1周的脑白质损伤检出率分别为1.67%、0和0。研究组早产儿出生24 h的血清NSE、S100β、IL-6水平均高于对照组(P均<0.05);研究组早产儿出生72 h的血清NSE、S100β水平均高于对照组(P均<0.05)。相比四项分别单独检测,四项联合检查的曲线下面积(AUC)值最大,其诊断灵敏度、特异度最高,分别为94.32%和93.54%。结论新生儿颅脑超声联合血清NSE、S100β、IL-6检测能有效提高早期诊断早产儿脑白质损伤的敏感性,可作为临床诊断早产儿脑白质损伤的有效手段。Objective To investigate the value of neonatal brain ultrasound combined with serum neuron specific enolase(NSE),S100βand interleukin-6(IL-6)in the diagnosis of brain white matter injury in preterm infants.Methods 60 premature infants with and without brain white matter injury were selected as the study group and the control group,respectively.Serum NSE,S100β,IL-6 and brain ultrasound were performed.The results of the examination were compared and the diagnostic efficacy of the combination of the four examinations in the brain white matter injury of premature infants were analyzed.Results The detection rate of brain white matter injury in the study group was 90.00%at 24 hours after birth,86.67%at 72 hours after birth,85.00%at one week after birtn,and 1.67%,0 and 0 at 24 hours,72 hours and one week after birth in the control group.The levels of NSE,S100βand IL-6 in the study group were significantly higher than those in the control group at 24 hours after birth(P all<0.05),while the levels of NSE and S100βin the study group were significantly higher than those in the control group at 72 hours after birth(P all<0.05).Compared with the four independent tests,the AUC value of the four combined tests was the highest,with the highest diagnostic sensitivity and specificity of 94.32%and 93.54%respectively.Conclusion Ultrasound examination of newborn brain combined with serum NSE,S100β,IL-6 can effectively improve the sensitivity of early diagnosis of brain white matter injury in preterm infants,which can be used as an effective means of clinical diagnosis of brain white matter injury in preterm infants.
