二巯丙磺酸钠治疗慢性汞中毒的效果及对血清sIL-2R、IL-6、TNF-α水平的影响

摘　　要：目的探讨二巯丙磺酸钠治疗慢性汞中毒的效果及对血清可溶性白细胞介素-2受体(sIL-2R)、白介素-6(IL-6)、肿瘤坏死因子α(TNF-α)水平的影响。方法选取西安市中心医院自2016年12月至2019年12月收治的71例慢性汞中毒患者,按用药方案分为对照组(35例)和观察组(36例)。对照组给予二巯丁二酸胶囊口服治疗,观察组给予5%二巯丙磺酸钠250 mg肌内注射治疗,比较两组患者临床疗效、尿汞值、血清细胞因子水平、肾功能指标及不良反应。结果观察组患者总有效率(100.00%)高于对照组(85.71%)(P<0.05);治疗1个疗程、2个疗程后,观察组患者尿汞值均低于对照组(P均<0.05);治疗2个疗程后,观察组患者血清sIL-2R、IL-6、TNF-α水平及血肌酐(Scr)、血尿素氮(BUN)、尿蛋白水平均降低,且观察组均低于对照组(P均<0.05);两组不良反应差异无统计学意义(P>0.05)。结论二巯丙磺酸钠治疗慢性汞中毒具有理想的驱汞作用,可下调血清sIL-2R、IL-6、TNF-α水平,改善肾功能。Objective To investigate the effect of sodium dimercaptosulphonate on chronic mercury poisoning and its effects on serum soluble interleukin-2 receptor(sIL-2R),interleukin-6(IL-6),tumor necrosis factor alpha(TNF-α)levels.Methods 71 patients with chronic mercury poisoning treated in our hospital from December 2016 to December 2019 were selected and divided into control group(35 cases)and observation group(36 cases)according to the medication regimen.The control group was treated with oral dimercaptosuccinic acid capsule and the observation group was treated with intramuscular injection of 5%sodium dimercaptosulphonate 250 mg.The efficacy,urinary mercury value,cytokine levels,renal function index and adverse reactions were compared between the two groups.Results The total effective rate in the observation group(100.00%)was higher than that in the control group(85.71%)(P<0.05).After one course and two courses of treatment,the urinary mercury value in the observation group was lower than that in the control group(P all<0.05).After two courses of treatment,the levels of serum sIL-2R,IL-6,TNF-α,serum creatinine(Scr),blood urea nitrogen(BUN),and urine protein in the observation group were all decreased,and the levels in the observation group were lower than those in the control group(P all<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion Sodium dimercaptosulphonate has an ideal effect of removing mercury in the treatment of chronic mercury poisoning.It can lower serum sIL-2R,IL-6 and TNF-αlevels and improve renal function.
