传统螺钉内固定术与非刚性内固定术对闭合性踝关节骨折伴下胫腓联合损伤患者的效果

摘　　要：目的:研究临床治疗闭合性踝关节骨折伴下胫腓联合损伤疾病患者应用传统螺钉内固定术和非刚性内固定术的治疗效果。方法:从本院收治的闭合性踝关节骨折伴下胫腓联合损伤疾病患者中抽取60例进行试验分析,收治时间为2008年1月-2018年1月,根据治疗方法的不同将患者分为参考组(n=30)和试验组(n=30),参考组实施传统螺钉内固定术,试验组实施非刚性内固定术疗法,比较两组踝关节功能、下胫腓间隙、治疗效果优良率、术后并发症情况及生活质量。结果:随访2年后,试验组的踝关节功能评分、下胫腓间隙指标均优于参考组,差异有统计学意义(P<0.05);试验组生活质量评分,即总体健康、心理健康、精神健康、活力等分值均高于参考组,差异有统计学意义(P<0.05);试验组临床治疗效果优良率为96.67%,较参考组高,差异有统计学意义(P<0.05);试验组术后并发症情况总发生率为6.67%,低于参考组,差异有统计学意义(P<0.05)。结论:相较于传统螺钉内固定术疗法,非刚性内固定术治疗方案的疗效更杰出,应用于闭合性踝关节骨折伴下胫腓联合损伤疾病时的效果更显著,能够更好地改善患者踝关节功能,对生活质量的恢复及提高方面起到推动作用,值得于临床上实施广泛推广。Objective:To study the clinical effect of traditional screw internal fixation and non rigid internal fixation in the treatment of closed ankle fracture with lower tibiofibular syndesmosis injury.Method:Sixty patients with closed ankle fracture and lower tibiofibular syndesmosis injury in our hospital were selected for experimental analysis.The treatment time was from January 2008 to January 2018.According to different treatment methods,patients were divided into reference group(n=30)and experimental group(n=30).Traditional screw internal fixation was performed in the reference group,and non-rigid internal fixation was performed in the experimental group.Ankle function,lower tibiofibular space,excellent and good rate of treatment effect,postoperative complications and quality of life were compared between the two groups.Result:After 2 years of follow-up,the ankle function score and lower tibiofibular space index of the experimental group were better than those of the reference group(P<0.05).The scores of overall health,psychology health,mental health and vitality in the experimental group were significantly higher than those in the reference group(P<0.05).The excellent and good rate of the experimental group was 96.67%,which was higher than that of the reference group(P<0.05).The total incidence of postoperative complications in the experimental group was 6.67%,which was significantly lower than that in the reference group(P<0.05).Conclusion:Compared with the traditional screw internal fixation,the curative effect of non rigid internal fixation is more outstanding,and it is more effective in the treatment of closed ankle fracture with lower tibiofibular syndesmosis injury,which can better improve the ankle function of patients and promote the recovery and improvement of quality of life,which is worthy of wide promotion in clinical practice.
