摘  要:目的探讨生物钟基因脑-骨骼肌ARNT样基因1(Bmal1)和周期基因2(Per2)在慢性阻塞性肺疾病急性加重(AECOPD)患者外周血单个核细胞(PBMCs)中的表达及临床意义。方法回顾性分析40例AECOPD患者(观察组)和40例门诊随访慢性阻塞性肺疾病(COPD)稳定期患者(对照组)的临床资料,比较两组PBMCs中Bmal1、Per2、外周血Th17、调节性T细胞(Treg)、IL-6及TNF-α的表达,分析生物钟基因与免疫功能及炎性因子指标的相关性。结果观察组生物钟基因Bmal1和Per2 mRNA表达低于对照组(P<0.01),而Th17、Treg、IL-6和TNF-α表达高于对照组(P<0.01)。AECOPD患者PBMCs中Bmal1、Per2与Th17、Treg、IL-6、TNF-α表达均呈负相关(P<0.05)。结论 AECOPD患者PBMCs中生物钟基因Bmal1和Per2表达下降,且与免疫功能紊乱、炎症反应密切相关。以生物钟基因为靶点的时辰治疗有望成为慢性气道疾病治疗新途径。Objective To explore the expression and clinical significance of clock genes brain and muscle ARNT-like gene 1(Bmal1) and period gene 2(Per2) in peripheral blood mononuclear cells(PBMCs) of the patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods The clinical data of 40 AECOPD patients(group A) and 40 patients with chronic obstructive pulmonary disease(COPD) in stable phase(group B) were retrospectively analyzed.The levels of Bmal1,Per2 in PBMCs, peripheral blood Th17,regulatory T cells(Treg),IL-6 and TNF-α were compared between the two groups.The correlation of clock genes with immune function and inflammatory factor index was analyzed.Results The expressions of clock genes Bmal1 and Per2 mRNA in group A were significantly lower than those in group B(P<0.01), while the expressions of Th17,Treg, IL-6 and TNF-α in group A were higher than those in group B(P<0.01).The expressions of Bmal1 and Per2 in PBMCs of AECOPD patients were negatively correlated with Th17,Treg, IL-6 and TNF-α(P<0.05).Conclusion The expressions of clock genes Bmal1 and Per2 in PBMCs of AECOPD patients are decreased, which is closely related to immune dysfunction and inflammation.Chronotherapy targeted on clock genes is expected to be a new approach for chronic airway disease.
