摘  要:目的探讨纳米磁微粒化学发光法(NM-CLIA)在检测系统性红斑狼疮(SLE)相关自身抗体中的应用价值。方法采用NM-CLIA和线性免疫印迹法(LIA)检测108例SLE患者自身抗体,比较两种方法在检测自身抗体的一致性。采用Spearman相关性分析抗体NM-CLIA定量结果与SLE疾病活动性指数(SLEDAI)评分的相关性。结果 NM-CLIA和LIA在检测抗nRNP抗体、抗核糖体蛋白P0抗体、抗Ro-52抗体、抗Ro-60抗体和抗SSB抗体结果一致性较好(Kappa≥0.75),抗Sm抗体和抗双链DNA抗体结果一致性一般(0.40≤Kappa<0.75),抗核小体抗体和抗组蛋白抗体结果一致性较差(Kappa<0.40)。抗双链DNA抗体、抗核小体抗体、抗组蛋白抗体、抗Sm抗体与SLEDAI评分呈正相关(r=0.334、0.362、0.323、0.201,P<0.05)。结论 NM-CLIA可用于检测SLE相关自身抗体,定量结果能够反映SLE疾病活动度。Objective To investigate the application value of nanoparticle magnetic chemiluminescent immunoassay(NM-CLIA) in the detection of systemic lupus erythematosus(SLE)-related autoantibodies.Methods The autoantibodies were detected in 108 SLE patients by NM-CLIA and line immunoassay(LIA).The consistency between NM-CLIA and LIA for SLE-related autoantibodies was compared.The relationship of quantitative results of autoantibodies and score of SLE disease activity index(SLEDAI) was analyzed by Spearman analysis.Results NM-CLIA and LIA showed good consistency in detecting anti-nRNP antibody, anti-ribosomal protein P0 antibody, anti-Ro-52 antibody, anti-Ro-60 antibody and anti-SSB antibody(Kappa≥0.75).The consistency of anti-Sm antibody and anti-double-stranded DNA antibody was general(0.40≤Kappa<0.75).The consistency of anti-nucleosome antibody and anti-histone antibody was poor(Kappa<0.40).Anti-double-stranded DNA antibody, anti-nucleosome antibody, anti-histone antibody and anti-Sm antibody were positively correlated with SLEDAI score(r=0.334,0.362,0.323 and 0.201,P<0.05).Conclusion NM-CLIA can be used to detect the SLE-related autoantibodies.The quantitative results can reflect the activity of SLE.
