摘 要:目的探讨沙利度胺联合吡非尼酮治疗肺间质纤维化患者的临床效果及对血清TNF-α、TGF-β1、IGF-Ⅰ水平的影响效果。方法选取本院2017年1月至2021年10月收治的肺间质纤维化患者100例,按照随机数字法分为两组,每组50例。对照组采用吡非尼酮治疗,研究组采用沙利度胺联合吡非尼酮治疗。比较两组细胞因子指标(血清TNF-α水平、血清TGF-β1水平、血清IGF-Ⅰ水平)、治疗效果以及肺功能指标(FVC、FEV_1、DLCO)。结果研究组细胞因子指标均低于对照组(P<0.05);研究组治疗总有效率高于对照组(P<0.05);研究组肺功能指标优于对照组(P<0.05)。结论应用沙利度胺联合吡非尼酮治疗肺间质纤维化患者,疗效显著,可有效改善肺间质纤维化患者的细胞因子水平,不良反应较少,安全性较高,可明显改善肺功能,值得临床上进一步应用推广。Objective To investigate the clinical effect of thalidomide combined with pirfenidone on patients with pulmonary interstitial fibrosis and the serum levels of TNF-α, TGF-β1 and IGF-Ⅰ. Methods A total of 100 patients with pulmonary fibrosis admitted to our hospital from January2017 to October 2021 were selected and they were divided into two groups by random number method, with 50 cases in each group. The control group was treated with pirfenidone, while the study group was treated with thalidomide combined with pirfenidone. The cytokines levels(serum TNF-α levels, serum TGF-β1 level, serum IGF-Ⅰ levels), therapeutic effects and lung function indicators(FVC, FEV1, DLCO) were compared between two groups. Results The cytokine index of the study group was lower than that of the control group(P<0.05). The efficacy of the study group was higher than that of the control group(P<0.05). The lung function index of the study group were better than those of the control group(P<0.05). Conclusion In the treatment of patients with pulmonary interstitial fibrosis, the application of thalidomide combined with pirfenidone has obvious curative effect and can effectively improve the cytokine level of patients with pulmonary interstitial fibrosis. Meanwhile, it has fewer adverse reactions, high safety, and significantly improves the pulmonary function, which is worthy of further application and promotion in clinical practice.
