板型胸骨固定系统与爪型胸骨固定系统在心脏手术中胸骨固定中应用效果比较

摘　　要：目的:对比板型胸骨固定系统与爪型胸骨固定系统在心脏手术胸骨固定中的应用效果。方法:选取2016年4月—2018年3月期间于我院行正中开胸心脏手术的110例患者,利用随机数表法分为两组,各55例。对照组采用板型胸骨固定系统,观察组采用爪型胸骨固定系统,对比两组手术成功率、术后恢复情况、术后疼痛程度及并发症发生率。结果:两组患者手术成功率均为100%;但观察组胸骨闭合时间、胸腔引流管放置时间、住院时间均较对照组短,术后第一个24 h胸引流少于对照组,VA S评分(2.83±1.12)分,低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率(3.64%)低于对照组的14.55%,差异有统计学意义(P<0.05)。结论:板型胸骨固定系统与爪型胸骨固定系统在心脏手术胸骨固定中均可取得良好效果,而爪型胸骨固定系统更利于提升胸骨闭合速度,术后引流量少,引流管放置时间短,可促进患者术后恢复,减轻患者疼痛,降低术后并发症发生率。Objective To compare the effect of plate sternum fixation system and claw sternum fixation system in sternum fixation in cardiac surgery.Methods A total of 110 patients who underwent central thoracotomy in our hospital from April 2016 to March 2018 were selected and divided into two groups with 55 patients in each group by random number table method.The control group was treated with plate sternum fixation system,while the observation group was treated with claw sternum fixation system.The success rate,postoperative recovery,postoperative pain and complication rate of the two groups were compared.Results The success rate of operation in both groups was 100%.However,the observation group was shorter than the control group in terms of sternal closure time,placement time of thoracic drainage tube and length of hospital stay.The thoracic drainage in the first 24 hours after surgery was less than that in the control group,and the VAS score(2.83±1.12)was lower than that in the control group,with statistical significance(P<0.05).The incidence of complications in the observation group(3.64%)was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Both plate sternum fixation system and claw sternum fixation system can achieve good results in sternum fixation in cardiac surgery,while claw sternum fixation system is more beneficial to improve the speed of sternum closure,reduce postoperative drainage,promote postoperative recovery,relieve pain,and reduce the incidence of postoperative complications.
