摘 要:目的:探讨应用外耳矫形器对婴幼儿先天性耳廓畸形无创矫正的时机以及矫治疗效,以期在该地区的临床应用。方法:选择2020年7月—2021年2月云浮市妇幼保健院耳鼻喉科的32例(41耳)月龄为3个月内的先天性外耳廓畸形的婴幼儿,采用外耳矫形器(某国产品牌的新型综合式矫形器)替代传统的外科胶带或绷带、弧形矫形器和夹子式耳模等,应用非手术耳模无创矫正技术改善畸形耳的耳轮、对耳轮、对耳轮上下脚、三角窝、耳舟及耳甲腔等部位的畸形。矫治期间,定期跟踪随访记录,密切观察矫治效果等。矫治完成后,矫正效果根据改善程度分为优、良、差3个等级。结果:32例(41耳)先天性外耳廓畸形患儿均顺利完成耳模无创矫治,包括:招风耳5耳、杯状耳3耳、垂耳5耳、环缩耳7耳、耳轮畸形6耳、猿耳5耳、隐耳3耳、耳甲异常凸起3耳、复合耳畸形2耳、Ⅱ度Ⅰ型耳廓结构畸形2耳。期间1耳出现皮肤破溃、5耳胶布粘贴处出现皮疹,经过对症治疗后均能继续佩戴,结果畸形耳均得到不同程度的改善,外耳矫形器治疗耳廓畸形的成功率为95%(结果为优或良)。结论:早期(出生后3个月内)采取外耳矫形器矫治婴幼儿先天性外耳廓形态畸形、隐耳和Ⅰ度外耳廓结构畸形的矫治成功率高,矫治疗效取决于外耳廓畸形的严重程度以及开始矫治的时间。耳模无创矫治外耳廓畸形的时间开始越早(出生后1周内),矫治的效果越好,外耳矫形器佩戴的时间则越短。新生儿听力筛查在出生后24~48 h内进行,同时对外耳廓畸形做出同步排查,可在最早期发现外耳廓畸形,从而避免漏诊和耽误最佳的矫治时间窗。Objective To explore the opportunity and curative effect of applying External ear orthosis to non-invasive correction of congenital auricle deformity in infants,with a view to its clinical popularization and application in this area.Methods From July,2020 to February,2021,32 infants(41 ears)with congenital external auricle deformity within 3 months were treated in the Department of Otolaryngology,Yunfu Maternal and Child Health Hospital.External ear orthosis(a new comprehensive orthosis of domestic brand)was used instead of traditional surgical tape or bandage,curved orthosis and clip-on ear mold,and non-surgical ear mold non-invasive correction technology was applied to improve the helix,anti-helix and ear mold of deformed ears During the treatment,follow-up records were kept regularly,and the treatment effect was closely observed.After correction,the correction effect can be divided into three grades:excellent,good and poor according to the degree of improvement.Results Thirty-two children(41 ears)with congenital external auricle deformity have successfully completed the non-invasive treatment of the ear mold,including 5 ears of the wind ear,3 ears of the cup ear,5 ears of the vertical ear,7 ears of the constricted ear,6 ears of the helix deformity,5 ears of the ape ear,3 ears of the hidden ear,3 ears of the abnormal protrusion of the concha,2 ears of the compound ear deformity,and 2 ears of theⅡⅠtype auricle structure deformity.During this period,skin ulceration occurred in one ear and skin rash occurred in five ears,which could be worn continuously after symptomatic treatment.Conclusion All deformed ears were improved to varying degrees,and the success rate of External ear orthosis in treating auricle deformity was 95%(the result was excellent or good).ConClusions Early(within 3 months after birth),External ear orthosis has a high success rate in correcting congenital external auricle shape deformity,concealed ear and first degree external auricle structure deformity of infants,and the curative effect dep
