摘  要:目的:探究尿肾损伤分子-1(尿KIM-1)、尿表皮生长因子(尿EGF)联合检测在早产儿围生期急性肾损害中的应用价值。方法:本文以随机抽取法选择90例早产儿参与研究,按照暴露因素进行分组,包括胎龄28~31周30例(28~31周组)、胎龄32~34周30例(32~34周组)、胎龄35~37周30例(35~37周组),另选取30例足月健康新生儿作为对照组。在新生儿出生后24 h,72 h及生后1 w采集尿液3ml用酶联免疫法检测尿KIM-1、尿EGF。结果:28~31周组、32~34周组、35~37周组的尿KIM-1、尿EGF水平表现出逐渐降低的情况,28~31周组、32~34周组、35~37周组的尿KIM-1、尿EGF水平较对照组新生儿明显升高,差异有统计学意义(P<0.05)。结论:尿KIM-1、尿EGF在早产儿围生期急性肾损伤的早期诊断工作中有较高的参考价值。早期联合应用多种指标对早产儿进行肾功能的监测,可为肾损伤的早期防治提供依据。Objective To explore the application value of urinary kidney injury molecule-1(urine KIM-1)and urine epidermal growth factor(urine EGF)in preterm infants with perinatal acute kidney damage.Method 90 cases of preterm infants were randomly selected to participate in the study.The preterm infants were divided into gestational age 28~31 weeks group(30 cases),gestational age 32~34 weeks group(30 cases),gestational age 35~37 weeks group(30 cases)according to exposure factors,and 30 full-term healthy newborns were selected as the control group.Urine samples were collected at 24 hours,72 hours and 1 week after birth.KIM-1 and EGF were detected by enzyme-linked immunosorbent assay(ELISA).Results The levels of urine KIM-1 and EGF in gestational age 28~31 weeks group,gestational age 32~34 weeks group and gestational age 35~37 weeks group were decreased gradually.The levels of urine KIM-1 and urinary EGF in gestational age 28~31 weeks group,gestational age 32~34 weeks group and gestational age 35~37 weeks group were significantly higher than those in the control group(P<0.05).Conclusion Urine KIM-1 and EGF have high reference value in the early diagnosis of perinatal acute kidney injury in preterm infants.Early combined application of multiple indicators to monitor renal function of premature infants can provide basis for early prevention and treatment of renal injury.
