摘 要:目的比较S-P入路与Ganz入路治疗Pipkin骨折的临床效果。方法选取宝鸡市人民医院收治的40例Pipkin骨折患者,根据手术入路不同将其分为S-P组(20例,S-P入路)和Ganz组(20例,Ganz入路)。比较两组的手术相关指标、髋关节功能及并发症发生情况。结果两组的术中出血量、手术时间、骨折愈合时间、术后1周疼痛评分、髋关节功能优良率及并发症总发生率比较,差异无统计学意义(P>0.05)。Ganz组术前和术后1 d的血红蛋白差值(ΔHb)明显高于S-P组(P<0.05)。结论S-P入路与Ganz入路均为治疗Pipkin骨折的良好选择。Ganz入路显露充分、便于操作,适用于所有类型的Pipkin骨折;S-P入路本身对于股骨头血供无明显破坏,但在治疗PipkinⅣ型骨折时对于后方的骨折块显露、固定存在一定困难,长时间牵拉以及过度剥离会对股骨头血供产生损伤。Objective To compare the clinical effects of S-P approach and Ganz approach in the treatment of Pipkin fracture.Methods Forty patients with Pipkin fracture admitted in the people's hospital of Baoji were selected and divided into S-P group(20 cases,S-P approach)and Ganz group(20 cases,Ganz approach)according to different surgical approaches.The operation related indexes,hip joint function and complications were compared between the two groups.Results There were no significant differences in the amount of intraoperative blood loss,operation time,fracture healing time,pain score at 1 week after operation,excellent and good rate of hip joint function and total incidence of complications between the two groups(P>0.05).The difference of hemoglobin before operation and at 1 day after operation(ΔHb)in the Ganz group was significantly higher than that in the S-P group(P<0.05).Conclusion Both the S-P approach and Ganz approach are good choices for the treatment of Pipkin fracture.The Ganz approach is fully exposed and easy to operate,which is suitable for all types of Pipkin fractures;the SP approach has no significant damage to the blood supply of the femoral head,but it is difficult to expose and fix the posterior fracture during the treatment of Pipkin typeⅣfractures,and prolonged traction and excessive stripping will damage the blood supply of the femoral head.
