摘　　要：目的探讨鼓室内注射地塞米松联合盐酸氨溴索治疗分泌性中耳炎的临床效果及对白介素-2(IL-2)、白介素-6(IL-6)、白三烯D4(LTD4)水平和血管通透性的影响。方法选取2018年7月至2020年7月收治的分泌性中耳炎患者90例,以随机数字表法将其分为对照组(45例,鼓室内注射地塞米松)和观察组(45例,鼓室内注射地塞米松+盐酸氨溴索)。比较两组的治疗效果。结果观察组的治疗总有效率为93.33%,高于对照组的75.56%(P<0.05)。治疗后,观察组的IL-2、IL-6、LTD4水平低于对照组(P<0.05)。治疗后,观察组的透明质酸(HA)、血小板活化因子(PAF)水平低于对照组,纤维连接蛋白(Fn)水平高于对照组(P<0.05)。结论鼓室内注射地塞米松联合盐酸氨溴索治疗分泌性中耳炎的临床效果显著,可促进IL-2、IL-6及LTD4水平的降低,改善血管通透性,值得临床应用和推广。Objective To explore the clinical effect of intratympanic injection of dexamethasone combined with ambroxol hydrochloride in the treatment of secretory otitis media and its influence on the levels of interleukin-2(IL-2),interleukin-6(IL-6)and leukotriene D4(LTD4)and vascular permeability.Methods A total of 90 patients with secretory otitis media treated from July 2018 to July 2020 were divided into control group(45 cases,intratympanic injection of dexamethasone)and observation group(45 cases,intratympanic injection of dexamethasone+ambroxol hydrochloride)according to random number talde method.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was 93.33%,which was higher than 75.56%in the control group(P<0.05).After treatment,the levels of IL-2,IL-6 and LTD4 in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of hyaluronic acid(HA)and platelet activating factor(PAF)in the observation group were lower than those in the control group,and the level of fibronectin(Fn)was higher than that in the control group(P<0.05).Conclusion Intratympanic injection of dexamethasone combined with ambroxol hydrochloride in the treatment of secretory otitis media has a significant clinical effect,it can promote the reduction of IL-2,IL-6 and LTD4 levels and improve vascular permeability,which is worthy of clinical application and promotion.
