摘  要:目的应用剪切波弹性成像(SWE)对正常乳腺内脂肪、腺体与皮下脂肪硬度进行评价,探讨其临床应用价值。资料与方法前瞻性选取行乳腺检查的115名女性正常志愿者,均为静止期乳腺。测量不同年龄段、不同月经周期受试者的乳腺内脂肪、腺体与皮下脂肪的剪切波速度(SWV),并比较其差异。结果乳腺腺体、乳腺内脂肪、皮下脂肪平均SWV分别为(1.442±0.180)m/s、(1.238±0.155)m/s、(1.164±0.107)m/s,不同部位SWV比较,差异有统计学意义(F位置=114.630,P<0.001)。20~29岁女性腺体平均SWV为(1.415±0.166)m/s,30~39岁女性腺体平均SWV为(1.471±0.174)m/s,40~49岁女性腺体平均SWV为(1.472±0.206)m/s,>50岁女性腺体平均SWV为(1.406±0.173)m/s。育龄期(20~49岁)女性腺体平均SWV随年龄增加而逐渐增大,围绝经期(>50岁)女性腺体平均SWV小于育龄期女性。分泌期腺体平均SWV为(1.475±0.191)m/s,增殖期腺体平均SWV为(1.448±0.191)m/s,月经期腺体平均SWV为(1.384±0.159)m/s,分泌期腺体平均SWV最大,略大于增殖期,月经期腺体SWV平均值最小。结论SWE能定量评价正常成年女性乳腺内脂肪、腺体与皮下脂肪的硬度,为临床发现乳腺疾病提供有力的证据,具有一定的临床应用价值。Purpose To explore the assessment value of shear wave elastography(SWE)in the stiffness of the normal intramammary fat,mammary gland and subcutaneous fat in clinical practice.Materials and Methods A total of 115 healthy females who underwent breast examination were selected,and all breasts were at rest,excluding pregnancy and lactation.Shear wave elastic velocity(SWV)measurements of intramammary fat and gland,and subcutaneous fat in different ages and menstrual cycles were compared.Results The average SWV of breast gland,fat and subcutaneous fat were(1.442±0.180)m/s,(1.238±0.155)m/s and(1.164±0.107)m/s,respectively.There were significantly differences in SWV of breast gland,fat and subcutaneous fat among the three groups(Fposition=114.630,P<0.001).The average SWV of the gland in 20-29 years old,30-39 years old,40-49 years old and>50 years old female were(1.415±0.166)m/s,(1.471±0.174)m/s,(1.472±0.206)m/s and(1.406±0.173)m/s,respectively.The gland average SWV of childbearing age(20-49 years old)female gradually increases with age,that in per-meniscus female(>50 years old)was smaller than that in childbearing age.The average SWV of the secretory phase,proliferating phase and menstrual period were(1.475±0.191)m/s,(1.448±0.191)m/s and(1.384±0.159)m/s,respectively.The average SWV of the gland in secretory phase was slightly higher than that in the proliferating phase,and the menstrual period had the lowest SWV.Conclusion SWE could quantitatively evaluate the stiffness of intramammary fat,mammary gland and subcutaneous fat in adult normal women,which could provide the objective imaging evidence for breast diseases,and has a certain value in clinical practice.
