摘 要:目的探讨无创呼吸机联合氧气雾化吸入治疗危重症支气管哮喘的效果。方法从2018年5月—2020年5月选取84例危重症支气管哮喘患者,随机抽签分为对照组与观察组,各42例,对照组给予常规雾化吸入治疗,观察组给予无创呼吸机联合氧气雾化吸入治疗,对比治疗效果、血清炎性因子、血气指标情况。结果观察组与对照组治疗总有效率分别为(95.24%)及(73.81%),差异有统计学意义(χ^(2)=7.372,P<0.05);治疗后,两组白细胞介素-6(IL-6)及白细胞介素-8(IL-8)均降低,观察组IL-6(132.12±21.45)、IL-8(105.23±24.61)ng/L低于对照组(145.34±21.52)、(155.12±26.78)ng/L,差异有统计学意义(t=2.820、8.890,P<0.05);治疗后,两组动脉血氧分压(PaO_(2))升高,动脉血二氧化碳分压(PaCO_(2))下降,观察组PaO_(2)(85.23±3.56)mmHg、PaCO_(2)(43.81±5.23)mmHg优于对照组(70.27±3.61)、(57.23±4.28)mmHg,差异有统计学意义(t=19.122、12.869,P<0.05)。结论无创呼吸机联合氧气雾化吸入治疗危重症支气管哮喘效果良好,血气指标改善,血清炎性因子水平降低,具有推广价值。Objective To explore the effect of non-invasive ventilator combined with oxygen atomization inhalation in the treatment of critically ill bronchial asthma.Methods From May 2018 to May 2020,84 patients with critically ill bronchial asthma were selected and randomly divided into a control group and an observation group with 42 cases each.The control group was given conventional aerosol inhalation therapy,and the observation group was given a non-invasive ventilator combined with oxygen nebulized inhalation therapy,compared the therapeutic effects,serum inflammatory factors,blood gas indicators.Results The total effective rates of treatment in the observation group and control group were 95.24%and 73.81%,respectively,and the difference was statistically significant(χ^(2)=7.372,P<0.05);after treatment,the two groups of interleukin-6(IL-6)and interleukin IL-8(IL-8)decreased,the observation group IL-6(132.12±21.45)ng/L,IL-8(105.23±24.61)ng/L were lower than the control group(145.34±21.52)ng/L,(155.12±26.78)ng/L,the difference was statistically significant(t=2.820,8.890,P<0.05);after treatment,the arterial blood oxygen partial pressure(PaO_(2))increased,and the arterial blood carbon dioxide partial pressure(PaCO_(2))decreased in the two groups.The observation group PaO_(2)(85.23±3.56)mmHg,PaCO_(2)(43.81±5.23)mmHg was better than the control group(70.27±3.61)mmHg and(57.23±4.28)mmHg,the difference was statistically significant(t=19.122,12.869,P<0.05).Conclusion Non-invasive ventilator combined with oxygen atomization inhalation has a good effect in the treatment of critically ill bronchial asthma,blood gas indicators are improved,and serum inflammatory factors are reduced,which is of promotional value.
