螺旋CT尿路造影和静脉尿路造影诊断泌尿系梗阻性疾病的效果和安全性评价

摘　　要：目的探讨分别采取螺旋CT尿路造影(CTU)、静脉尿路造影(IVU)用于临床诊断泌尿系梗阻性疾病的价值及安全性。方法方便选取2017年1月—2019年4月该院收治的经手术病理检查确诊为泌尿系梗阻性疾病的80例患者为研究对象,对研究对象采取使用不同的检查方式,主要进行IVU检查与CTU检查,根据检查方式不同做对照,比较不同的检查方式的诊断效率、诊断价值及不良反应发生情况。结果采取CTU方式的检查时间为(19.26±2.56)min,明显短于IVU方式(64.25±6.12)min的检查时间,对比差异有统计学意义(t=60.659,P<0.001);CTU检查方式对泌尿系梗阻性疾病的总检出率为98.75%,显著高于IVU检查方式81.25%的总检出率,对比差异有统计学意义(χ^(2)=13.611,P<0.001);CTU检查方式检查后不良反应总发生率为2.50%,明显低于IVU检查11.25%的总发生率,对比差异有统计学意义(χ^(2)=4.783,P=0.029)。结论在临床诊断泌尿系梗阻性疾病中,采取CTU检查在诊断效率、诊断效果及安全性上均优于IVU检查,值得推广。Objective To explore the value and safety of spiral CT urography(CTU)and intravenous urography(IVU)for clinical diagnosis of urinary tract obstructive diseases.Methods Conveniently selected 80 patients diagnosed with urinary obstructive diseases through surgical pathological examinations admitted from January 2017 to April 2019 as the research subjects in the hospital.Different examination methods were adopted for the subjects,mainly IVU examination and CTU examination,make comparisons according to different inspection methods to compare the diagnostic efficiency,diagnostic value and occurrence of adverse reactions of different inspection methods.Results The inspection time of CTU method was(19.26±2.56)min,which was significantly shorter than the inspection time of IVU method(64.25±6.12)min.The difference was statistically significant(t=60.659,P<0.001);The total detection rate of obstructive diseases of the urinary tract by CTU inspection method was 98.75%,which was significantly higher than the total detection rate of 81.25%in the IVU inspection method,and the difference was statistically significant(χ^(2)=13.611,P<0.001);after the CTU inspection method,the total incidence of adverse reactions was 2.50%,which was significantly lower than the total incidence of 11.25%in IVU examinations.The difference was statistically significant(χ^(2)=4.783,P=0.029).Conclusion In the clinical diagnosis of urinary tract obstructive diseases,CTU examination is superior to IVU examination in terms of diagnostic efficiency,diagnostic effect and safety,and is worthy of promotion.
