眼用凝胶在鼻内镜下鼻腔泪囊造口术中的应用

摘　　要：目的探讨眼用凝胶在鼻内镜下鼻腔泪囊造口术中的应用效果。方法选择宜春市第二人民医院2017年1月—2020年11月行鼻内镜下鼻腔泪囊造口术治疗的46例慢性泪囊炎患者纳入本次研究,按照术中操作不同分为眼用凝胶组25例和泪道探针组21例,泪道探针组暴露泪囊后应用泪道探针支撑泪囊再继续手术,眼用凝胶组暴露泪囊后经上泪点将眼用凝胶注入泪囊支撑再继续手术。比较2组暴露泪囊后操作时间、术中出血量、手术效果以及术后并发症情况。结果眼用凝胶组暴露泪囊后操作时间及术中出血量均低于泪道探针组,差异显著(P<0.05);眼用凝胶组术后3个月总有效率均略高于泪道探针组,但差异不显著(P>0.05);眼用凝胶组术后并发症总发生率低于泪道探针组,差异显著(P<0.05)。结论鼻内镜下鼻腔泪囊造口术中使用眼用凝胶能够缩短手术时间、降低并发症风险,值得临床推荐。Objective To investigate the effect of ophthalmic gel on nasal dacryocystorhinostomy under nasal endoscope.Methods 46 patients with chronic dacryocystitis who underwent endoscopic dacryocystostomy in our hospital from January 2017 to November 2020 were included in this study.According to the intraoperative operation,they were divided into 25 groups:ophthalmic gel group and lacrimal duct probe group(21 cases).The lacrimal probe group exposes the lacrimal sac and uses lacrimal probe to support the lacrimal sac and continues the operation.The eye gel group exposes the lacrimal sac to the lacrimal sac through the upper lacrimal sac to support the lacrimal sac and continues the operation.The operation time,intraoperative blood loss,operation effect and postoperative complications were compared between the two groups.Results The operation time and blood loss of eye gel group were lower than that of lacrimal probe group,and there was a significant difference between the two groups(P<0.05).The cure rate and total effective rate of eye gel group were slightly higher than that of lacrimal probe group at 2 months and 3 months after operation,but there was no significant difference between the two groups(P>0.05).The incidence of postoperative complications in ophthalmic gel group was lower than that in lacrimal probe group,and there was a significant difference between the two groups(P<0.05).Conclusion The use of eye gel in nasal cavity dacryocystomy under nasal endoscope can shorten the operation time and reduce the risk of complications,which is worthy of clinical recommendation.
