体外冲击波碎石术治疗上尿路结石的临床观察

摘　　要：目的探讨上尿路结石患者采用体外冲击波碎石术治疗的临床效果。方法选取2018年4月—2019年6月收治的103例上尿路结石患者,按治疗方法不同分为2组,对照组56例行输尿管镜取石术治疗,观察组47例行体外冲击波碎石术治疗。比较2组患者结石清除率、并发症、治疗费用以及肾功能指标[血清肌酐(SCr)、血尿素氮(BUN)]。结果观察组结石清除率(97.87%)高于对照组(83.93%);观察组的住院费用显著低于对照组(P<0.05)。观察组并发症率4.26%,显著低于对照组的23.21%(P<0.05)。手术前,对比2组肾功能指标,差异不明显(P>0.05);手术后,观察组SCr水平、BUN水平低于对照组(P<0.05)。结论上尿路结石患者采用体外冲击波碎石术治疗,可有效清除结石,减少治疗费用,改善肾功能,且并发症少,值得临床推广。Objective To investigate the clinical outcomes obtained after completion of treatment with extracorporeal shock wave lithotripsy for patients with upper urinary tract stones.Methods A total of 103 patients with upper urinary tract stones who were treated from April 2018 to June 2019 were selected and divided into two groups according to their treatment methods,while in the control group,56 patients were treated by ureteroscopic lithotomy;In the observation group,47 patients were treated with extracorporeal shock wave lithotripsy;Stone clearance,complications of treatment,cost of treatment,and measures of renal function[SCr(serum creatinine),bun(blood urea nitrogen)]were compared between the two groups.Results The stone clearance rate of the observation group(97.87%)was higher than that of the control group(83.93%);Hospital costs were significantly lower in the observation group than in the control group(P<0.05).The complication rate of 4.26%in the observation group was significantly lower than the 23.21%in the control group(P<0.05);Before surgery,the renal function indexes of the two groups were compared,and the difference was not significant(P>0.05);After surgery,SCr level and BUN level were lower in the observation group than in the control group(P<0.05).Conclusions In patients with upper urinary tract stones treated with extracorporeal shock wave lithotripsy,it can effectively clear stones and reduce the cost of treatment,and it is safe and reliable with few complications,which merits clinical promotion.
