透视定位双内侧入路关节镜下跖腱膜松解和骨质增生清除术治疗顽固性跟痛症的疗效分析

摘　　要：目的探讨透视定位双内侧入路关节镜下跖腱膜松解和骨质增生清除术治疗顽固性跟痛症(PHS)的疗效。方法选择2017年1月—2020年6月安远北方医院收治的24例顽固性PHS患者,均行透视定位双内侧入路关节镜下跖腱膜松解和骨质增生清除术治疗。比较术前、术后6个月疼痛程度,美国足踝外科协会踝-后足评分系统(AOFAS)评分,患者满意度,并发症发生率。结果术后6个月视觉模拟评分法(VAS)评分低于术前,AOFAS评分高于术前,均有统计学差异(P<0.05);随访期间无明显并发症发生;术后患者满意度为91.67%。结论透视定位双内侧入路关节镜下跖腱膜松解和骨质增生清除术治疗顽固性PHS具有较高的安全性,可减轻患者疼痛程度,改善足踝功能,患者满意度高。Objective To investigate the effect of arthroscopically positioned double medial approach for the treatment of refractory calcaneal pain(PHS)with decompression of plantar fascia and bone hyperplasia.Methods Twenty-four patients with refractory PHS admitted to Anyuan North Hospital from January 2017 to June 2020 were treated with arthroscopic metatarsal fasciolysis and bone hyperplasia under fluoroscopic positioning and dual medial approach.The pain level before and 6 months after the operation,the American Association of Foot and Ankle Surgery Ankle-Hindfoot Score System(AOFAS),postoperative patient satisfaction,and complication rate were compared.Results The visual analogue scale(VAS)score was lower than that of the preoperative 6 months after operation,and the AOFAS score was higher than that of the preoperative.There was a statistical difference(P<0.05);no obvious complications occurred during the follow-up period;the postoperative patient satisfaction was 91.67%.Conclusion The fluoroscopy positioning of the dual medial approach arthroscopic plantar fasciotomy and bone hyperplasia debridement has high safety in the treatment of refractory PHS.It can reduce the patient's pain and improve the ankle function.The patient's satisfaction is high.
