摘　　要：在我国医药卫生体制改革和国防与军队改革双重改革形势下,军队医院参与军队改革和国家医改势在必行。分析了某军队医院在公立医院改革、分级诊疗推进、医保基金严格监管、薪酬制度改革、医师多点执业准入等新形势下面临的对外医疗药品零差价、财政补贴无政策支撑、技术竞争压力凸显、人才保留困难等问题,针对这些问题,提出了新形势下的军队医院对外医疗策略,包括:加强政策研究,拓展军民融合发展;重视人才培养,保持特色优势;开展公益医疗,提升应急救援能力;适应医保监管,规范诊疗行为;优化服务流程,提高患者满意度。通过这些措施进一步提升卫勤保障和对外医疗服务水平,保持军队医院可持续健康发展。In the context of both health system reform and national defense and military reform,it is imperative for military hospitals to participate in military reform and national health care reform.This article analyzes the problems and challenges for military hospitals under the new situations of public hospital reform,promotion of hierarchical diagnosis and treatment systems,strict supervision of medical insurance funds,reform of compensation system,and multi-site practice access of physicians.The problems included zero price difference of drugs prescribed to civil patients and the lack of policy support in fiscal subsidies.In addition,the competition in medical market,pressure from technical competition,and difficulties in talent retention are also issues to be addressed.In response to these problems,countermeasures and solutions for providing medical service to civil community by the military hospitals were proposed,including:strengthening policy research and promoting the civil-military integration;attaching importance to talent training and maintaining the unique advantages;carrying out public welfare medical service,especially the emergency rescue;compliance with medical insurance supervision and standardizing treatment behavior;and optimizing the service process to improve patient satisfaction.These measures may further improve the health security and medical services of military hospitals for the civil society and ensure the sustainable and healthy development of military hospitals.
