茵陈柴胡汤对胆囊结石合并胆总管结石患者肝胆功能的影响

摘　　要：目的研究茵陈柴胡汤对胆囊结石合并胆总管结石患者肝胆功能的影响。方法选取108例胆囊结石合并胆总管结石患者,均于2019年2月—2020年4月被收治,依据随机数字表法将其分为对照组与试验组,每组54例患者。其中对照组患者于经腹胆双镜术后采用常规护肝及营养支持治疗,试验组在对照组基础上联合茵陈柴胡汤治疗,以7 d为一个疗程,2组均治疗2个疗程。比较2组治疗后临床疗效,围术期情况及治疗前后肝胆功能水平变化。结果治疗后,试验组患者临床总有效率为92.59%,对照组患者临床总有效率为77.78%,试验组比对照组升高;与对照组相比,试验组患者的肛门排气时间、住院时间均缩短,且术中出血量减少;治疗后,所有患者TBA、AST、GPT水平较治疗前均降低,且试验组比对照组降低(均P <0.05)。结论茵陈柴胡汤可有效缓解胆囊结石合并胆总管结石患者的临床症状,并缩短其治疗时间,提高临床疗效,同时可减轻肝脏损伤,改善肝胆功能,促进康复。Objective To study the effect of Yinchen Chaihu decoction on hepatobiliary function in patients with gallbladder stones and common bile duct stones. Methods 108 patients with gallbladder stones and common bile duct stones were admitted to our hospital from February 2019 to April 2020 were selected. According to the random number table method,they were divided into control group and experimental group,and each group contains 54 patients. The control group was treated with conventional liver protection and nutritional support after transabdominal choledochoscopy. The experimental group was treated with Yinchen Chaihu decoction on the basis of the control group,with 7 days as a course of treatment,and both groups were treated for 2 courses. The clinical efficacy,perioperative conditions and changes in hepatobiliary function indexes before and after treatment were compared between the two groups. Results After comparison,the total clinical effective rate of patients in the experimental group was 92. 59%,the total clinical effective rate of the control group was77. 78%,and the experimental group was higher than the control group. Compared with before,the levels of TBA,AST and GPT of the two groups of patients decreased after treatment,and the experimental group was lower than the control group( all P < 0. 05). Conclusion Yinchen Chaihu decoction can effectively relieve the clinical symptoms of patients with choledocholithiasis,shorten the treatment time,improve the clinical efficacy,reduce liver damage,improve liver and gallbladder function,and promote recovery.
