摘  要:目的分析2018-2020年成都市某社会办医疗机构门诊就诊患者特点、服务状况、卫生服务需求及利用情况,发现本地区社会办医疗机构服务的主要特点及存在问题。方法收集2018-01/2020-12成都市某社会办医疗机构的患者就诊数据,同时,对同期初诊患者通过问卷调查的方式收集口腔卫生服务及利用的相关信息。了解就诊患者的特征、就诊疾病的分布,就诊人数的时间分布,以及就诊患者口腔卫生服务的需求、利用及满意度。采用SPSS 22.0软件进行χ^(2)检验。结果共收集到8602例患者资料,女性占比(59.8%)高于男性(40.2%);年龄最小1岁,最大者94岁,平均为47.0±20.6岁;职业构成以工人为主(59.8%);初诊者占94.9%;治疗费用支付方式主要为自费(99.5%)。就诊占比最高的前五位疾病分别为慢性牙龈炎(21.20%)、牙髓炎(17.75%)、阻生牙(17.53%)、急性牙周炎(12.42%)、龋齿(9.53%)。上述五类疾病在不同年龄、不同职业、不同就诊类型之间疾病的分布差异有统计学意义(P<0.05)。从就诊方式来看,除牙髓炎、龋齿主要为复诊外,其余疾病均为初诊比例较高。2018-2020年就诊人数呈现上升趋势,7-9月为就诊高峰。患者定期口腔检查者占19.6%,定期洗牙者占17.2%;最为困扰的口腔问题主要为牙龈出血(50.5%)。选择该机构就诊的原因主要为交通便利(31.2%)、等候时间短(26.4%)。患者对治疗效果、收费以及医护的技术服务满意度均超过85%。等候时间超过30分钟者仅占0.4%。结论该社会办医疗机构就诊患者具有特殊的分布特点,主要就诊的疾病与全人群流行的疾病谱接近,医护人员人数的配置基本合理,医护的服务技术及水平基本上能够得到患者认可,而交通便利及等候时间较短等方面是该医疗机构与公立医疗机构相比具有优势的方面。Objective To analyze the characteristics,service status,health service demand and utilization of outpatients in a social medical institution in Chengdu from 2018 to 2020,and to find out the main characteristics and existing problems of the service of the social medical institutions in this region.Methods The clinical data of patients in a social medical institution in Chengdu from January 2018 to December 2020 was collected.Meanwhile,the information about oral health service and utilization was collected by questionnaire survey from the newly diagnosed patients in the same period.The characteristics of patients,the distribution of diseases,the time distribution of the number of patients,and the demand,utilization and satisfaction of oral health services of patients were analyzed.Chi-square test was used by SPSS 22.0 software.Results A total of 8602 patients were included in the analysis.The proportion of female patients(59.8%)was higher than that of male patients(40.2%).The age ranged from 1 year to 94 years,with an average of 47.0±20.6 years.The occupation composition was mainly workers(59.8%).The firstvisit patients accounted for 94.9%.The main way to pay for the treatment expenses was private(99.5%).The top five diseases were chronic gingivitis(21.20%),pulpitis(17.75%),impacted teeth(17.53%),acute periodontitis(12.42%)and dental caries(9.53%).There were significant differences in the distribution of the above five diseases among different ages,different occupations and different types of treatment(P<0.05).From the perspective of the way of treatment,except pulpitis and dental caries,the proportion of the first-visit patients of the other diseases was higher than that of the fellow-up patients.From 2018 to 2020,the number of patients showed an upward trend,and the peak was from July to September.The patients with regular oral examination accounted for 19.6%and regular tooth washing for 17.2%.The most common oral problem was gingival bleeding(50.5%).The main reasons for choosing this institution were convenien
