摘　　要：溃疡性结肠炎(UC)是一种慢性复发性炎症性肠病,其特征是结直肠持续的黏膜炎症。内镜下黏膜愈合被认为是UC的主要治疗目标,然而黏膜愈合并不一定意味着组织学愈合。UC组织学缓解比内镜下缓解有更好的临床预后,因此需要一个经过验证且通用的组织学评分系统。Nancy指数是最新提出且已经过验证的组织学指数,由"溃疡、急性炎症细胞浸润和慢性炎症细胞浸润"3个组织学项目组成,定义了5个疾病活动等级"0~4级"。Nancy指数对UC患者的组织学活动性具有预后价值,有潜力成为临床实践和临床试验的通用组织学工具。该文旨在综述Nancy指数在评价UC黏膜愈合的价值及其应用。Ulcerative colitis(UC)is a chronic recurrent inflammatory bowel disease,which is characterized by persistent mucosal inflammation in the colorectal system.Endoscopic mucosal healing is considered to be the main therapeutic goal of UC.However,mucosal healing does not necessarily represent histological healing.Compared with endoscopic remission,histological remission of UC yields better clinical prognosis.A validated universal histological grading system is urgently required.Nancy index,the latest validated histological index,consists of three histological items of ulcer,acute inflammatory cell infiltration and chronic inflammatory cell infiltration,defining five histological disease activity ranging from grade 0-4.Nancy index has prognostic value for histological disease activity in UC patients,which has the potential to become a universal histological tool for clinical practice and clinical trials.This article aims to review the application value of Nancy index in evaluating mucosal healing in UC patients.
