摘 要:目的:分析湿润烧伤膏对点阵CO2激光治疗烧伤后瘢痕增生术后创面修复效果及疼痛的影响。方法:选取笔者医院2017年1月-2018年12月收治的烧伤后瘢痕增生患者98例,根据治疗方法不同分为两组。两组均给予瘢痕内切除薄化和点阵CO2激光治疗,对照组术后给予常规应用凡士林油纱、无菌纱布和硅酮凝胶膏加压包扎,观察组术后给予湿润烧伤膏加无菌纱布加压包扎。比较两组创面愈合时间、细菌转阴时间、色素沉着率、视觉模拟评分(Visual analogue scale,VAS)、温哥华瘢痕量表(Vancouver scar scale,VSS)评分及血管内皮细胞生长因子(Vascular endothelial growth factor,VEGF)、转化生长因子β1(Transforming growth factor-β1,TGF-β1)水平。结果:观察组创面愈合时间、细菌转阴时间短于对照组,色素沉着率低于对照组,差异具有统计学意义(P<0.05)。治疗1d、3d、7d时,观察组VAS评分低于对照组,差异具有统计学意义(P<0.05)。治疗1个月、3个月时,观察组VSS评分低于对照组,差异具有统计学意义(P<0.05)。治疗7d时,观察组VEGF、TGF-β1水平低于对照组,差异具有统计学意义(P<0.05)。结论:湿润烧伤膏应用于点阵CO2激光治疗烧伤后瘢痕增生术后可抑制血清VEGF、TGF-β1的表达水平,缓解疼痛,促进创面愈合和瘢痕消退,减少色素沉着。Objective To analyze the effect of MEBO on wound healing and pain after dot matrix CO2 laser treatment of with scar hyperplasia after burn.Methods 98 patients with post burn scar hyperplasia treated in our hospital from January 2017 to December 2018 were selected,and divided into two groups according to the treatment methods.Both groups were treated with intrascar resection and thinning and dot matrix CO2 laser.The control group was given vaseline gauze,sterile gauze and silicone gel paste after operation,and the observation group was given moist burn ointment and sterile gauze after operation.The wound healing time,bacterial negative time,pigmentation rate,Visual analogue scale(VAS),Vancouver scar scale(VSS),VEGF and TGF-β1 level were compared between the two groups.Results The wound healing time and bacterial negative conversion time in the observation group were shorter than those in the control group,and the pigmentation rate was lower than that in the control group,the differences were statistically significant(P<0.05).The VAS score of the observation group was lower than that of the control group at 1d,3d and 7d of treatment(P<0.05).At 1 and 3 months of treatment,the VSS score of the observation group was lower than that of the control group(P<0.05).After 7 days of treatment,the level of VEGF and TGF-β1 in the observation group was lower than that of the control group(P<0.05).Conclusion MEBO can inhibit serum VEGF and TGF-β1 in the treatment of post burn scar hyperplasia by dot matrix CO2 laser,relieve pain,promote wound healing and scar regression,and reduce pigmentation.
