改良保留上睑浅层回流静脉的切开法重睑成形术疗效探讨

摘　　要：目的:探讨一种改良的保留上睑浅层回流静脉的切开法重睑成形术的临床效果。方法:回顾分析2019年1月-2020年1月行改良保留上睑浅层回流静脉的切开法重睑成形术的36例就医者临床资料。根据常规切开术式改良,减少局部麻醉药注射量,在显露保留上睑浅层回流静脉的基础上去除部分眼轮匝肌与多余眶隔脂肪,应用微型钨针精细止血,将切口缘皮肤与睑板前筋膜缝合。结果:36例就医者均对手术效果满意,术后即刻局部肿胀轻微,术后6d肿胀明显消退,随访1~6个月,所有就医者重睑形态自然。结论:改良保留上睑浅层回流静脉的切开法重睑成形术对手术解剖结构的显露和组织保留较好,创伤小,术后肿胀轻、恢复快,术后重睑形态自然,具有临床推广应用价值。Objective To investigate the clinical effect of improved blepharoplasty with incision method preserving the superficial reflux vein of upper eyelid.Methods The clinical data of 36 patients who underwent modified blepharoplasty from January 2019 to January 2020 were analyzed retrospectively.The operation procedure was improved from the routine incision method.The injection amount of lidocaine was reduced.Part of the orbicularis oculi muscle and excess orbital fat were removed,based on exposing and retaining the superficial regurgitant vein.Mini-tungsten needle was used to stop the bleeding.The conjunctional fascial tissue over tarsal plate was sutured with the skin.Results All the 36 patients were satisfied with the effect of the operation.The swelling was slight immediately after operation,and the swelling subsided significantly 6 days after operation.Followed-up for 1 to 6 months,the double eyelid shape was natural.Conclusion Modified blepharoplasty with preservation of superficial regurgitant vein has the advantages of good exposure of anatomical structure and tissue preservation,with less trauma,mild postoperative swelling,rapid recovery,and natural double-eyelid shape.
