摘 要:目的探讨三维可视化分析辅助肩袖损伤治疗的临床效果。方法选择2020年1—10月赣州市人民医院骨科治疗的40例肩袖损伤患者,按随机数字表法将其分为2组,各20例。观察组采用基于X线计算体层摄影(CT)与核磁共振(MRI)扫描重建肩部骨、肌肉与肌腱的3D模型及三维可视化辅助关节镜下肩袖损伤修复术治疗,对照组采用常规CT与MRI辅助关节镜下肩袖损伤修复术治疗。比较2组术中指标、疼痛程度、肩关节功能,并评价冈上肌萎缩与术后功能康复之间的关系。结果观察组手术时间(79.53±10.16)min,短于对照组的(92.13±12.58)min,术后VAS评分(3.09±0.54)分,低于对照组的(4.12±0.73)分,均有统计学差异(P<0.05);2组术中出血量、手术前后Constant-Murley评分对比,无统计学差异(P>0.05);经Logistic回归分析显示肌肉萎缩程度为影响患者术后功能康复的独立危险因素(P<0.05且OR≥1)。结论三维可视化技术辅助关节镜下肩袖修补术治疗具有精准化、个体化等特点,有利于缩短手术耗时,减轻患者疼痛程度,患者术后肩关节功能恢复良好。Objective To explore the clinical effect of 3D visualization analysis of rotator cuff injury diagnosis and treatment.Methods From January to October 2020,40 patients with rotator cuff injury treated in our hospital were selected and divided into two groups according to the random number table method,with 20 cases in each group.The observation group used computed tomography(CT)and magnetic resonance(MRI)scans to reconstruct 3D models of shoulder bones,muscles and tendons,and three-dimensional visualization assisted arthroscopic repair of rotator cuff injuries.The control group used conventional CT With MRI-assisted arthroscopic repair of rotator cuff injury.Intraoperative indexes,pain degree,shoulder joint function were compared between the two groups,and the relationship between supraspinatus atrophy and postoperative functional rehabilitation was evaluated.Results The operation time of the observation group was(79.53±10.16)min,which was shorter than the control group(92.13±12.58)min,and the postoperative VAS score was(3.09±0.54)points,which was lower than the control group(4.12±0.73)points,which was statistically significant.Difference(P<0.05);There was no statistical difference between the two groups in intraoperative blood loss and Constant-Murley score before and after surgery(P>0.05);Logistic regression analysis showed that the degree of muscle atrophy was an independent risk affecting the functional recovery of patients after surgery Factors(P<0.05 and OR≥1).Conclusion The three-dimensional visualization technology-assisted arthroscopic rotator cuff repair treatment has the characteristics of precision and individualization,which is beneficial to shorten the operation time,reduce the pain of the patient,and the patient's shoulder joint function recovers well.
