自体角膜缘上皮移植术治疗翼状胬肉的临床效果观察

摘　　要：目的研究自体角膜缘上皮移植术治疗翼状胬肉的临床效果。方法选取2019年3月—2020年12月丰顺县人民医院收治的80例翼状胬肉患者,采用随机数字表法分为对照组与观察组,每组各40例。对照组行翼状胬肉摘除术,观察组行翼状胬肉摘除术配合自体角膜缘上皮移植术,对比分析2组患者的临床治疗效果、血清免疫学指标[血清免疫球蛋白A(IgA)、血清免疫球蛋白M(IgM)、血清免疫球蛋白G(IgG)]、血清血管内皮生长因子(VEGF)、内皮抑素(Endostatin)水平及复发率。结果观察组治疗总有效率95.00%,高于对照组的77.50%(P<0.05);治疗后,观察组IgA、IgM、IgG水平明显低于对照组,VEGF水平低于对照组,Endostatin水平高于对照组(P<0.05);观察组复发率2.63%,低于对照组的25.81%(P<0.05)。结论对翼状胬肉患者开展自体角膜缘上皮移植术的效果理想,可改善相关血清免疫学指标,患者术后恢复速度较快,复发率低。Objective To study the clinical effect of limbal autograft transplantation in the treatment of pterygium and its effect on the level of serum immunological indicators in patients.Methods A total of 80 patients with pterygium admitted to our hospital from March 2019 to December 2020 were selected as the research objects,and were randomly divided into control group and observation group by number table method,with 40 cases in each group.The control group underwent pterygium extraction,and the observation group underwent pterygium extraction combined with limbal epithelial autotransplantation.To compare and analyze the clinical treatment effects,serum immunological indicators[serum immunoglobulin A(IgA),serum immunoglobulin M(IgM),serum immunoglobulin G(IgG)],serum vascular endothelial growth factor(VEGF),Endostatin(Endostatin)levels and disease recurrence rate.Results The total effective rate of the observation group was 95.00%higher than that of the control group 77.50%(P<0.05);After treatment,the observation group's IgA,IgM,and IgG levels were significantly lower than those of the control group,VEGF levels were lower than the control group,and Endostatin levels were higher than the control group(P<0.05);The disease recurrence rate of 2.63%in the observation group was lower than 25.81%in the control group(P<0.05).Conclusion Limbal epithelial autotransplantation for patients with pterygium has an ideal therapeutic effect and has a good effect on related serum immune levels.The patient has a faster postoperative recovery and a lower recurrence rate.
