摘  要:目的分析1例临床患者产生抗-Ec合并抗-Lea的血清学及交叉配血特点,初步探讨患者血液管理在临床疑难交叉配血实践中的应用。方法用血型血清学方法对患者标本进行ABO、Rh、Lewis血型鉴定试验,直接抗球蛋白试验,抗体筛查及抗体鉴定试验,抗体效价测定试验等。结果经血型血清学试验确认患者血清中存在抗-Ec合并抗-Lea。选择ABO血型同型,Ec、Lea抗原阴性献血者血样与患者血样进行交叉配血,配血相合,患者输血后无输血不良反应发生。结论患者产生了抗-Ec合并抗-Lea混合抗体,在临床输血时要选择对应抗原阴性的血进行交叉配血,解决了临床疑难交叉配血患者的输血问题。Objective The serological and cross-match characteristics of a clinical patient with anti-Ec and anti-Lea were analyzed to explore the application of patient blood management mode in clinical difficult cross-matching practice.Methods ABO,Rh,Lewis blood group identification test,direct antiglobulin test,antibody screening and antibody identification test,and antibody titer test were performed on the patients'samples by blood group serological method.Results The presence of anti Ec and Lea was confirmed by blood group serological test.The blood samples of the donors with the same ABO blood group and negative Ec and Lea antigens were selected for cross matching with the patients'blood samples,and there was no adverse reaction after blood transfusion.Conclusion The patient developed mixed antibodies:anti-Ec and anti-Lea.In clinical blood transfusion,the blood with negative antigen should be selected for cross matching,so as to solve the problems of blood transfusion in patients with difficult cross matching.
