保胆取石术后应用熊去氧胆酸或牛磺熊去氧胆酸对近期结石复发的影响对比

摘　　要：目的研究应用熊去氧胆酸(UDCA)或牛磺熊去氧胆酸(TUDCA)对保胆取石术近期结石复发的影响差异。方法选取2018年2月至2019年2月我院收治的进行保胆取石术的胆结石患者150例,用随机数字表法分为UDCA组和TUDCA组,每组75例,UDCA组术后应用熊去氧胆酸进行治疗,TUDCA组应用牛磺熊去氧胆酸进行治疗,进行为期1年的治疗及回访调查。比较两组的临床疗效,治疗前后两组胆囊壁厚度、胆囊收缩率及结石复发率。结果TUDCA组的临床总有效率明显高于UDCA组(P<0.05),治疗后两组胆囊壁厚度较治疗前显著减小(P<0.05),胆囊收缩程度显著增加(P<0.05);TUDCA组比UDCA组胆囊壁厚度减小更明显(P<0.05),胆囊收缩程度更大(P<0.05),且结石复发率明显更低(P<0.05)。结论与UDCA相比,保胆取石术后应用TUDCA疗效更显著,术后复发率更低。Objective To study the difference in the effect of ursodeoxycholic acid(UDCA)and tauroursodeoxycholic acid(TUDCA)on short-term stone recurrence after gallbladder-preserving cholecystolithotomy.Methods 150 patients with gallstones who had been treated with gallbladder-preserving cholecystolithotomy in the hospital between February 2018 and February 2019 were enrolled in the study.They were divided into UDCA group and TUDCA group by random number table method,75 cases in each group.After surgery,UDCA group and TUDCA group were treated with UDCA and TUDCA,respectively.1-year of treatment and follow-up was performed.Clinical effect,gallbladder wall thickness,gallbladder contraction rate and stone recurrence rate were compared between the two groups.Results The total clinical response rate of TUDCA group was significantly higher than that of UDCA group(P<0.05).After treatment,gallbladder wall thicknesses of both groups were significantly decreased(P<0.05),and gallbladder contraction was significantly increased(P<0.05).Changes of gallbladder wall thickness and gallbladder contraction were greater in TUDCA group than UDCA group(P<0.05),and stone recurrence rate was significantly lower than UDCA group(P<0.05).Conclusion Compared with UDCA,TUDCA is more effective after gallbladder-preserving cholecystolithotomy,and postoperative recurrence rate is lower.
