自动勾画软件LinkingMed和SPICE应用于头颈部危及器官勾画的准确性比较

摘　　要：目的:比较LinkingMed和SPICE 2种自动勾画软件勾画头颈部危及器官的准确性。方法:选取31例2019年10月至2020年4月在某院接受放疗的鼻咽癌患者的临床资料,由资深的放疗医师在Pinnacle39.10治疗计划系统上手动勾画眼球、晶体、视神经、脑干、脊髓、腮腺和下颌骨等危及器官,并采用LinkingMed和SPICE对其进行自动勾画。以手动勾画为金标准,计算并比较2种软件自动勾画结果的Dice相似性系数(Dice similarity coefficient,DSC)、Hausdorff距离(Hausdorff distance,HD)、包容性指数(inclusive index,IncI)和敏感性指数(sensitivity index,SI)。采用SPSS 19.0软件进行统计学分析。结果:眼球、晶体、视神经、脊髓和下颌骨的自动勾画结果中,LinkingMed组总体上优于SPICE组,除了眼球的IncI值、左晶体的HD值、左视神经的SI值、右视神经的DSC值以及下颌骨的HD值和SI值外,其他指标差异均有统计学意义(P<0.05)。左右腮腺和脑干的勾画结果中,LinkingMed组和SPICE组相似,除了脑干的DSC值、HD值和左腮腺的HD值、IncI值及SI值外,其他指标差异无统计学意义(P>0.05)。结论:对于头颈部危及器官的自动勾画,LinkingMed略优于SPICE。Objective To compare the accuracies of automatic contouring software LinkingMed and SPICE when used to outline head and neck organs at risk(OAR).Methods The clinical data of 31 patients with nasopharyngeal carcinoma treated with radiotherapy at some hospital from October 2019 to April 2020 were selected,and the OAR of the eye,crystal,optic nerve,brainstem,spinal cord,parotid gland and mandible were manually outlined on the Pinnacle39.10 treatment planning system by a senior radiotherapist and automatically sketched using LinkingMed and SPICE.The Dice similarity coefficient(DSC),Hausdorff distance(HD),inclusive index(IncI)and sensitivity index(SMI)were calculated and compared between the two software using manual outlining as the gold standard.SPSS 19.0 software was used for statistical analysis.Results LinkingMed generally outperformed SPICE in automatic outlining of the eyeball,crystal,optic nerve,spinal cord and mandible,with statistically significant differences(P<0.05)except for the IncI values of the eyeball,HD values of the left crystal,SI values of the left optic nerve,DSC values of the right optic nerve and HD and SI values of the mandible.LinkingMed and SPICE were similar in the outlining results of the right and left parotid glands and brainstem,with no statistically significant differences(P>0.05)except for the DSC and HD values of the brainstem and the HD,IncI and SI values of the left parotid gland.Conclusion LinkingMed is slightly better than SPICE for automatic outlining of head and neck OAR.
