双侧肌皮神经变异1例及临床意义

摘　　要：目的探讨双侧肌皮神经变异对上肢手术操作的影响,提供一定的解剖学基础。方法选取宜春学院人体解剖教研室实验室的一具成年女性尸体,对其采用局部解剖学方法,使其腋窝部分的臂丛神经充分暴露,观察各神经的解剖学形态并对比其对不同上肢手术的影响。结果左侧肌皮神经与正中神经存在交通支,且缺少喙肱支;右侧肌皮神经缺如,且外侧束和内侧束存在交通支,且正中神经缺少肱肌支。对比知道:肌皮神经位置及起止的变化对臂丛阻滞、肌皮神经的皮瓣移植等手术有一定的影响。结论该例肌皮神经变异对上肢手术操作有一定的影响。Objective To investigate the effect of bilateral musculocutaneous nerve variation on upper extremity surgery,and to provide anatomic basis.Methods An adult female cadaver was selected from the laboratory of Human Anatomy of Yichun University.The axillary brachial plexus was exposed using the local anatomy method to observe the anatomic morphology of each nerve and to compare its effect on different upper limb operations.Results The left musculocutaneous nerve and the median nerve had communicating branches and lacked the coracobrachial branch.The right musculocutaneous nerve was absent,the lateral and medial tracts had communicating branches,and the median nerve lacked the brachial branch.These results showed that the changes in musculocutaneous nerve position and starting and stopping had certain influence on brachial plexus block and musculocutaneous nerve flap transplantation.Conclusion Musculocutaneous nerve variation has certain influence on operation of upper limb.
