腹腔镜宫颈环扎术治疗宫颈机能不全的临床研究

摘　　要：目的:探讨非孕期行腹腔镜宫颈环扎术治疗宫颈机能不全的有效性及妊娠结局。方法:选取2013年6月至2020年8月收治的宫颈机能不全患者,17例行非孕期腹腔镜宫颈环扎术(研究组),33例孕12~16周患者行经阴道高位双带宫颈环扎术(对照组),统计分析两组患者临床资料及妊娠结局。结果:研究组与对照组手术时间[(43.47±9.62)min vs.(27.61±4.68)min]、术中出血量[(7.06±6.12)mL vs.(15.91±5.92)mL]、住院时间[(4.76±0.44)d vs.(7.21±0.49)d]差异有统计学意义(P<0.001),两组并发症发生率差异无统计学意义。研究组与对照组孕周≥37周的例数(17/17 vs.16/33)、出生体重[(3.36±0.29)kg vs.(3.06±0.45)kg]差异有统计学意义(P<0.05),转入新生儿重症监护室例数(0 vs.3)差异无统计学意义(P=0.200)。结论:非孕期行腹腔镜宫颈环扎术治疗宫颈机能不全可有效延长孕周,妊娠结局良好,值得临床推广应用。Objective:To investigate the efficacy and the pregnant outcomes of laparoscopic cervical ligation in the treatment of cervical insufficiency.Methods:Between Jun.2013 and Aug.2020,17 patients with cervical insufficiency who underwent non-pregnancy laparoscopic cervical ligation were selected as the study group,33 patients who underwent transvaginal high double-band cervical ligation during 12-16 weeks of gestation were selected as the control group.The clinical data and pregnancy outcomes of the two groups were statistically analyzed.Results:There were statistically significant differences in the operative time[(43.47±9.62)min vs.(27.61±4.68)min],the intraoperative blood loss[(7.06±6.12)mL vs.(15.91±5.92)mL],the hospital stay[(4.76±0.44)d vs.(7.21±0.49)d]between the two groups(P<0.001).The complications between the two groups were not significantly different.In the study group and control group,the gestational age≥37 weeks(17/17 vs.16/33)and the birth weight[(3.36±0.29)kg vs.(3.06±0.45)kg]were the statistically significantly different(P<0.05),there was no statistically significant difference in transfer to the neonatal intensive care unit(0 vs.3,P=0.200).Conclusions:Laparoscopic cervical ligation in the treatment of cervical insufficiency in non-pregnant state could effectively prolong the gestational week and confer good pregnant outcome,it is worthy of clinical application.
