摘 要:目的:观察电针、中药以及针药结合三种治疗方法对脑卒中肢体痉挛(spasticity of cerebral apoplexy,SCA)大鼠大脑海马区BDNF(脑源性神经营养因子)、TrkB(酪氨酸激酶B)及海马区神经突触超微结构的调节作用。方法:将实验大鼠随机分为空白组、假手术组、模型组、针药组、中药组、电针组每组各9只,采用改良Zea-Longa线栓法+内囊注射NMDA受体法制备脑卒中肢体痉挛大鼠模型,行为学评分确认模型成功后对电针组、中药组、针药组进行治疗,此外除空白组外各组均进行相同束缚及等量生理盐水灌胃。治疗后行为学检测神经功能损伤情况,通过透射电镜观察海马区突触的超微结构,采用逆转录-聚合酶链反应检测和蛋白质印迹法检测海马区BDNF、TrkB相关mRNA与蛋白的表达。结果:行为学观察Zea-longa神经功能评分,模型组与空白组比较评分增高,各治疗组评分降低;镜下观察模型组较空白组突触及囊泡数量减少,结构发生溶解;治疗组突触结构明显缓解,针药组作用最明显。模型组与空白组比较,BDNF、TrkBmRNA与蛋白表达降低,而治疗干预后与模型组比较各治疗组BDNF、TrkBmRNA与蛋白表达升高,针药组表达最高。结论:电针“曲池”“阳陵泉”及口服加味芍药甘草汤以及针药结合均可以缓解脑卒中肢体痉挛的神经损伤情况,其作用机制可能是调节大鼠海马区内BDNF、TrkB实现的,且针药结合最优。Objective:To observe the effects of three kinds of treatments,including electroacupuncture,modified Shaoyao Gancao Decoction,and combination of electroacupuncture and Shaoyao Gancao Decoction,on the regulation of brain-derived neurotrophic factor(BDNF),tyrosine kinase B(TrkB),and the ultrastructure of hippocampal neurosynapses in the hippocampus of rats with spasticity of cerebral apoplexy(SCA).Methods:The experimental rats were randomly divided into blank group,sham operation group,model group,electroacupuncture and prescription group,prescription group,and electroacupuncture group with nine rats in each group.The SCA model was established by the modified Zea-Longa injection NMDA receptor method,and the behavioral score was used for confirming the success of the model establishment.After that,each group received their corresponding treatments while the rats of the bland group were received the same restraint and gavage with saline.When all the treatments were finished,behavioral detection of neurological impairment was recorded,the ultrastructure of synapses in the hippocampus was observed by transmission electron microscopy,and the mRNA and proteins expressions of BDNF,TrkB related in hippocampus were detected by reverse transcription-polymerase chain reaction and Western blot.Results:Based on Zea-longa neurological function score,the model group had higher scores than the blank group.The scores in each treatment group were decreased.Under the microscope,the number of synapses and vesicles in the model group decreased when compared with the blank group.The synaptic structure of all the treatment groups were relieved,especially the electroacupuncture and prescription group.Compared with the blank group,the protein expressions of BDNF,TrkB mRNA in the model group decreased while the other three treatment groups’were increased.Besides,the levels of the mRNA and protein expressions in the electroacupuncture and prescription group was the highest.Conclusion:All three treatments can alleviate the SCA,especially the
