摘　　要：目的分析血清C-反应蛋白(CRP)/白蛋白(Alb)比值诊断糖尿病酮症酸中毒(DKA)合并感染的价值。方法选取2018年6月-2020年6月珠海市人民医院120例DKA患者作为研究对象,对其合并感染情况进行观察并根据是否发生感染分为感染组和未感染组。对两组患者的一般资料、入院常规实验室指标和血清CRP/Alb比值进行对比,采用受试者工作特征(ROC)曲线对常规实验室指标及血清CRP/Alb比值诊断DKA合并感染的价值进行分析。结果有55例患者发生感染,占45.83%。感染组患者入院时血清谷丙氨酸氨基转移酶(ALT)、CRP水平及血清CRP/Alb比值分别为(67.61±4.55)U/L、(73.44±10.80)mg/L和3.17±0.41,均高于未感染组,两组之间差异均有统计学意义(P<0.05)。ROC曲线分析结果显示,血清CRP/Alb比值及血清ALT、CRP水平诊断DKA合并感染曲线下面积(AUC)均有统计学意义(P<0.05),其中,血清CRP/Alb比值AUC最高,为0.913,在Cutoff值下,其灵敏度和特异度分别为90.90%和89.20%。结论DKA合并感染患者入院早期血清CRP/Alb比值呈现显著升高,临床医生可考虑将这一指标用于感染的辅助诊断。OBJECTIVE To analyze the value of serum C-reactive protein(CRP)/albumin(Alb)ratio in diagnosis of infection in the patients with diabetic ketoacidosis(DKA).METHODS A total of 120 DKA patients who were treated in Zhuhai People’s Hospital from Jun 2018 to Jun 2020 were recruited as the study subjects and divided into the infection group and the non-infection group according to the status of complication with infection.The baseline data,routine laboratory test indexes at admission and serum CRP/Alb ratio were compared between the two groups of patients,and the values of the routine laboratory test indexes and serum CRP/Alb ratio in diagnosis of DKA complicated with infection were analyzed by means of receiver operating characteristic(ROC)curve.RESULTS Totally 55 patients had infection,significantly accounted for 45.83%.The levels of serum alanine aminotransferase(ALT),CRP and serum CRP/Alb ratio of the infection group were respectively(67.61±4.55)U/L,(73.44±10.80)mg/L and(3.17±0.41)at the admission,higher than those of the non-infection group(P<0.05).ROC curve analysis showed that there was significant difference in the area under curve(AUC)in diagnosis of DKA complicated with infection between the serum CRP/Alb ratio and the serum ALT,CRP levels(P<0.05),the AUC of the serum CRP/Alb was the largest(0.913),and the sensitivity and specificity were respectively 90.90%and 89.20%cutoff values.CONCLUSION The serum CRP/Alb ratio of the DKA patients complicated with infection is remarkably elevated in the early stage at the admission,and clinicians may take it into account for the auxiliary diagnosis of infection.
