摘  要:目的探究乙型肝炎病毒(HBV)感染对2型糖尿病(T2DM)患者糖代谢以及外周血外泌体微小RNA(miRNA)水平的影响。方法以唐山市工人医院2015年7月-2019年7月收治98例T2DM合并HBV感染患者、94例单纯T2DM患者、同期体检100名健康者为研究对象,分别作为研究组、对照组以及健康组,测定三组糖代谢指标、外泌体miRNA以及HBV-DNA拷贝数指标,并分析其临床意义。结果糖代谢指标[空腹血糖(FPG)、空腹胰岛素(FINS)、餐后2 h血糖(2hPG)以及稳态模型胰岛素抵抗指数(HOMA-IR)]水平均为研究组>对照组>健康组(P<0.05);外泌体miR-122-5P、miR-29c-3P、miR-200c-3p表达量为研究组<对照组<健康组(P<0.05),miR-125a-5p表达量与以上趋势相反(P<0.05);研究组HBV-DNA阳性组患者糖代谢指标高于阴性组(P<0.05),外泌体miR-122-5P、miR-29c-3P、miR-200c-3p表达量低于阴性组(P<0.05),miR-125a-5p表达量高于阴性组;HBV-DNA拷贝数与糖代谢指标正相关(P<0.05),与miR-122-5P、miR-29c-3P、miR-200c-3p负相关(P<0.05),与miR-125a-5p正相关(P<0.05);糖代谢指标用于诊断T2DM患者HBV感染曲线下面积(AUC)分别为0.708、0.773、0.805、0.848;miR-122-5P、miR-125a-5p、miR-29c-3P、miR-200c-3p等外泌体miRNA用于诊断T2DM患者HBV感染AUC分别为0.993、0.743、0.957、0.989。结论T2DM患者胰岛素抵抗、外泌体miRNA水平与HBV感染发生以及HBV-DNA复制关系密切,对于诊断T2DM患者HBV感染具有一定价值。OBJECTIVE To explore the influence of hepatitis B virus(HBV)infection on glucose metabolism and peripheral blood exosomal microRNA(miRNA)in patients with type 2 diabetic mellitus(T2 DM).METHODS Totally 98 T2 DM patients complicated with HBV infection,94 patients with simple D2 DM and 100 healthy people who received physical examination in Tangshan Workers’Hospital from Jul 2015 to Jul 2019 were recruited as the study subjects and respectively divided into the study group,the control group and the healthy group.The glucose metabolism indexes,exosomal miRNA and HBV-DNA copies were determined,and the clinical significance was analyzed.RESULTS The levels of glucose metabolism indexes fasting blood glucose(FBG),fasting insulin(FINS)and 2 h postprandial glucose(2 hPG)as well as homeostasis model assessment of insulin resistance(HOMA-IR)index were significantly higher in the study group than in the control group than in the healthy group(P<0.05).The expression levels of exosomal miR-122-5 P,miR-29 c-3 P and miR-200 c-3 p were significantly lower in the study group than in the control group than in the healthy group(P<0.05),while the expression level of miR-125 a-5 p showed an opposite trend(P<0.05).In the study group,the glucose metabolism indexes of the HBV-DNA-positive patients were significantly higher than those of the HBV-DNA-negative patients(P<0.05),the expression levels of exosomal miR-122-5 P,miR-29 c-3 P and miR-200 c-3 p of the HBV-DNA-positive patients were significantly lower than those of the HBV-DNA-negative patients(P<0.05),while the expression level of miR-125 a-5 p of the HBV-DNA-positive patients was significantly higher than that of the HBV-DNA-negative patients(P<0.05).The HBV-DNA copies were positively correlated with the glucose metabolism indexes(P<0.05),negatively correlated with miR-122-5 P,miR-29 c-3 P and miR-200 c-3 p(P<0.05),and positively correlated with miR-125 a-5 p(P<0.05).The area under curves(AUCs)of the glucose metabolism indexes were respectively 0.708,0.773,0.805 and 0.848 in dia
