摘 要:目的研究能谱CT低剂量扫描对肺孤立小结节(SPN)的诊断价值。方法回顾性分析2017年10月至2019年10月本院收治的经病理学诊断证实的100例SPN患者的临床资料,根据病理学结果分为恶性组(n=47)和良性组(n=53),均经能谱CT低剂量扫描,比较两组碘基参数、水基参数、单能量CT值及能谱曲线斜率。结果恶性组ICLes_(Ap)、NIC_(Ap)、ICLes_(vp)、NIC_(vp)及ICD均高于良性组(P<0.05);两组WCLes_(Ap)、NWC_(Ap)、WCLes_(vp)、NWC_(vp)比较无显著差异(P>0.05);恶性组在40、70、100、140keV能量水平均高于良性组(P<0.05);动脉期和静脉期恶性组在40~70、70~100keV区间斜率高于良性组(P<0.05),两组在100~140keV区间斜率比较无显著差异(P>0.05)。结论碘基参数、单能量CT值及能谱曲线斜率有助于鉴别良恶性SPN,可为临床提供更准确的鉴别诊断信息。Objective To study the diagnostic value of energy spectrum CT low dose scanning in solitary pulmonary nodule(SPN).Methods The clinical data of 100 patients with SPN who were admitted to our hospital and confirmed by pathological diagnosis between October 2017 and October 2019 were retrospectively analyzed.They were divided into malignant group(n=47)and benign group(n=53)according to the pathological results.All the patients underwent energy spectrum CT low dose scanning.The iodine base parameters,water base parameters,single-energy CT value and the slope of the energy spectrum curve were compared between the two groups.Results ICLes_(Ap),NIC_(Ap),ICLes_(vp),NIC_(vp),and ICD in the malignant group were higher than those in the benign group(P<0.05).There was no significant difference in WCLes_(Ap),NWC_(Ap),WCLes_(vp),NWC_(vp) between the two groups(P>0.05).The energy levels of 40 keV,70 keV,100 keV,and 140 keV in malignant group were higher than those in the benign group(P<0.05).The interval slopes at 40~70 keV and 70~100 keV of malignant group in arterial and venous phases were higher than those in the benign group(P<0.05).However,there was no significant difference in interval slope at 100~140 keV between the two groups(P>0.05).Conclusion The iodine base parameters,single-energy CT value,and the slope of the energy spectrum cu rve are helpful for identifying benign and malignant SPNs,and can provide more accurate information for clinical diffe rential diagnosis.
