双源CT冠状动脉联合左房-肺静脉“一站式”成像检查的应用

摘　　要：目的探讨CT冠状动脉联合左房-肺静脉“一站式”成像检查的价值。方法75例冠状动脉粥样硬化的房颤患者通过CT“一站式”冠状动脉、左房-肺静脉成像,评价冠状动脉斑块狭窄程度,观察左房-肺静脉形态,测量肺静脉开口管径。结果检出冠状动脉粥样硬化症26例,冠心病47例,正常2例。左心房顶部平坦型17例,突起型49例,凹陷型9例。69例肺静脉为标准型,6例存在变异。同侧上肺静脉较下肺静脉增粗(P<0.05),男性左上肺静脉、右侧上下肺静脉较粗(P<0.05),左下肺静脉管径性别间无差异(P>0.05)。结论双源CT冠状动脉联合左房-肺静脉“一站式”成像是一种理想的影像学检查手段,避免了多次CT检查,对冠状动脉粥样硬化的房颤患者射频消融术前准备,术后随访有重要的临床价值。Objective To evaluate the value carried by“one-stop”imaging of coronary artery angiography combined with left atrial-pulmonary vein(PV)imaging under CT.Methods A total of 75 atrial fibrillation patients with coronary atherosclerosis were recruited to undergo“one-stop”imaging of coronary,left atrial-PV under CT to evaluate the severity of coronary plaque stenosis,observe the left atrial-PV morphology,and measure the diameter of the PV ostium.Results There were 26 patients diagnosed with coronary atherosclerosis,47 patients diagnosed with coronary heart disease,and 2 patients identified as normal.Besides,there were 17 patients found with flat left atrial roof,49 patients with a protrusion,and 9 patients with depression.Also,there were 69 patients with standard PVs and 6 patients showing variations.The ipsilateral superior PV was observed to be thicker than the inferior PV(P<0.05).The left superior PV and right superior and inferior PVs were found to be thicker among males(P<0.05).However,no significant difference was identified in left inferior PV diameter between genders(P>0.05).Conclusion The“one-stop”imaging of coronary,left atrial-PV under dual-CT is an ideal approach to medical imaging as it avoids the need for repeated CT examinations.It is of great clinical significance to preoperative preparation and postoperative follow-up of atrial fibrillation ablation for patients with coronary atherosclerosis.
