实时荧光PCR法与细菌培养法检测儿童粪便沙门菌的对比研究

摘　　要：目的比较实时荧光PCR法(RT-PCR)与细菌培养法检测粪便沙门菌的效果。方法回顾性分析2019年1月—2020年6月本院住院的158例腹泻患儿粪便中沙门菌RT-PCR法与细菌培养法的阳性率。结果 158例患儿中男性93例,女性65例,中位年龄为17个月。RT-PCR检测粪便沙门菌的阳性率为10.1%,粪便培养阳性率为17.1%,粪便培养阳性率高于RT-PCR法,差异有统计学意义(P <0.01);RT-PCR法相较于粪便培养法,其灵敏度为44.4%,特异度为96.9%。76例患儿在采样前已使用抗生素,使用抗生素后RT-PCR检测粪便沙门菌的阳性率为10.5%,粪便培养阳性率为23.7%,粪便培养阳性率仍高于RT-PCR法,差异有统计学意义(P <0.01)。结论目前RT-PCR法尚不能取代粪便培养的地位,进行粪便沙门菌PCR检查时,粪便细菌培养应同时进行,除非该PCR方法被证明其敏感性优于粪便培养。Objective Compare the effects of real-time PCR( RT-PCR) and bacterial culture in the detection of Salmonella in stool. Methods A retrospective analysis was performed to compare the positive rate of Salmonella by RT-PCR and bacterial culture in 158 children hospitalized from January 2019 to June 2020. Results Among the 158 children,93 were male and65 were female,with a median age of 17 months. The positive rate of Salmonella in feces by RT-PCR was 10. 1%,and17. 1% by bacterial culture. Bacterial culture showed higher positive rate than RT-PCR,with the difference statistically significant( P < 0. 01). Compared with the bacterial culture,the sensitivity and specificity of RT-PCR were respectively 44. 4%and 96. 9% respectively. 76 children were given antibiotics before sampling,the positive rate by RT-PCR was 10. 5%,and23. 7% by bacterial culture after antibiotic use. Bacterial culture still showed higher positive rate than RT-PCR,with the difference statistically significant( P < 0. 01). Conclusion RT-PCR canno’t replace bacterial culture in detecting Salmonella at present. Stool bacterial culture should be continued in parallel with real time PCR,unless the PCR method is proved to be more sensitive.
