摘  要:目的研究双相障碍Ⅰ型患者血清Hcy、叶酸、甲状腺激素及认知功能改变进行分析。方法前瞻性选取2017年1月—2019年10月于本院接受治疗的160例BDⅠ型患者作为BD组,另选取同期同年龄段健康体检者100例作为NC组。分别检测NC组以及BD组急性期、稳定期时的Hcy、FA、T_(3)、T_(4)、FT_(3)、FT_(4)、TSH、MoCA总分并进行对比,采用Pearson检验分析相关性。结果 NC组Hcy、T_(3)、T_(4)、FT_(3)、FT_(4)水平均低于BD组急性期,FA、TSH水平及MoCA总分均高于NC组,差异有统计学意义(P <0.05);随着BD组由急性期进入稳定期,其Hcy、T_(3)、T_(4)、FT_(3)、FT_(4)水平出现降低,但仍高于NC组,其FA、TSH水平及MoCA总分出现升高,但仍低于NC组,差异有统计学意义(P <0.05)。Pearson相关性分析表明,急性期BD患者MoCA总分与FA、TSH水平呈正相关(r=0.720、r=0.735,P <0.001),与Hcy、T_(3)、T_(4)、FT_(3)、FT_(4)水平呈负相关(r=-0.518、r=-0.342、r=-0.413、r=-0.382、r=-0.466,P <0.001);稳定期BD患者Mo CA总分与FA、TSH水平呈正相关(r=0.739、r=0.620,P <0.001),与Hcy、T_(3)、T_(4)、FT_(3)、FT_(4)水平呈负相关(r=-0.680、r=-0.290、r=-0.320、r=-0.239、r=-0.430,P <0.001)。结论 BD患者认知功能与FA、TSH水平呈正相关,与Hcy、T_(3)、T_(4)、FT_(3)、FT_(4)水平呈负相关。Objective To study the changes of serum Hcy,folic acid,thyroid hormones and cognitive function in patients with bipolar disorder type Ⅰ. Methods A total of 160 patients with BD Ⅰ type who were treated in our hospital from January 2017 to October 2019 were selected as the BD group,and 100 healthy patients of the same age at the same period were selected as the NC group. The total scores of Hcy,FA,T_(3),T_(4),FT_(3),FT_(4),TSH and MoCA in the acute phase and stable phase of the NC group and the BD group were detected and compared,and Pearson test was used to analyze the correlation. Results The levels of Hcy,T_(3),T_(4),FT_(3) and FT_(4) in the NC group were lower than that in the acute phase of the BD group,and the FA,TSH levels and MoCA total scores were higher than the NC group,with the differences statistically significant( P < 0. 05);With the transition from acute stage to stable stage,the levels of Hcy,T_(3),T_(4),FT_(3) and FT_(4) in BD group decreased,but were still higher than that in NC group;the levels of FA,TSH and total MOCA in BD group increased,but were still lower than that in NC group,with the differences statistically significant( P < 0. 05). Pearson correlation analysis showed that the total score of MOCA in patients with acute BD was positively correlated with FA and TSH levels( r = 0. 720,r = 0. 735,P < 0. 001),and negatively correlated with Hcy,T_(3),T_(4),FT_(3) and FT_(4) levels( r =-0. 518,r =-0. 342,r =-0. 413,r =-0. 382,r =-0. 466,P <0. 001). The total MOCA score of BD patients in stable stage was positively correlated with FA and TSH levels( r = 0. 739,r =0. 620,P < 0. 001),and negatively correlated with Hcy,T_(3),T_(4),FT_(3) and FT_(4) levels( r =-0. 680,r =-0. 290,r =-0. 320,r =-0. 239,r =-0. 430,P < 0. 001). Conclusion Cognitive function of BD patients is positively correlated with FA and TSH levels,and negatively correlated with Hcy,T_(3),T_(4),FT_(3) and FT_(4) levels.
