摘 要:目的探讨低能量剥脱性二氧化碳点阵激光用于儿童早期增生性瘢痕治疗的疗效及安全性。方法选取增生性瘢痕儿童患者15例,将其瘢痕平均分为A、B和C三组,A和C组随机分配为治疗组和对照组,治疗组每月进行一次低能量剥脱性二氧化碳点阵激光治疗,连续治疗3次,治疗后3个月和6个月分别随访。采用患者及观察者双向瘢痕评估量表(The Patient and Observer Scar Scale,POSAS)和视觉评估量表(Visual Analogue Scale,VAS)评估治疗结果。治疗组与对照组间的比较采用配对t检验,当P<0.05时认为数据具有统计学意义。结果POSAS总分、患者POSAS总分、观察者POSAS总分均为治疗组低于对照组(p<0.05)。单项评分中,除血管化评分外,其余各项评分均为治疗组更低(p<0.05)。血管化评分组间无显著性差异(p=0.064)。VAS平均评分为3.53±1.50。结论低能量剥脱性二氧化碳点阵激光可用于儿童早期增生性瘢痕的治疗。Objective To investigate the efficacy and safety of low-energy carbon dioxide ablative fractional laser in the treatment of early-stage pediatric hypertrophic scars.Methods 15 children with hypertrophic scars were enrolled.The scars were randomly divided into three groups:group A,B and C.Group A and C were randomly distributed into the treatment groups and control groups.Low-energy carbon dioxide ablative fractional laser was applied in the treatment groups once a month for 3 times.The follow-up time was 3 and 6 months after the treatment.Patient and observer scar assessment scale(POSAS)and visual analogue scale(VAS)were used to measure the outcome of the treatment.Results The total scores of POSAS,the total scores of patient POSAS as well as the total scores of observer POSAS of the treatment groups were significantly lower(p<0.05).Except vascularity,the other items were statistically different between the two groups,and the treatment groups were significantly lower.There was no significant difference in the vascularization score between the treatment groups and the control groups(p=0.064).The average VAS score was 3.53±1.50.Conclusion low-energy carbon dioxide ablative fractional laser can be used in the treatment of early-stage pediatric hypertrophic scars.
