摘　　要：目的探讨自体脂肪干细胞胶(SVF-gel)移植矫正半侧颜面萎缩的临床的应用。方法2017年7月至2021年6月,南方医科大学南方医院整形科共完成半侧颜面萎缩矫正术28例,年龄24~49岁,平均年龄28.42岁,其中患侧中、下面部矫正24例,患侧上、中、下面部均进行填充矫正者4例。手术方法采用我科室特色技术脂肪干细胞胶(SVF-gel)技术填充凹陷区域,以10%的过填充度为填充量。结果19例患者初次手术3个月后对手术效果满意,初次手术满意度达到67.86%;有7例进行了二次填充、2例进行了三次填充。并发症情况:所有患者单次填充量均超过20ml,未出现感染、液化、结节等并发症。远期随访填充区域外形良好。结论自体SVF-gel填充改善半侧颜面萎缩效果优于传统自体脂肪,术后恢复快、保留率有明显提高,并且可以通过冻存二次填充,减轻患者负担,值得临床推广应用。Objective To explore the clinical application of autologous adipose stromal vascular fraction gel(SVF-gel)transplantation to correct hemi-facial atrophy.Methods From July 2017 to June 2021,the Department of Plastic Surgery,Nanfang Hospital,Southern Medical University completed 28 cases of hemifacial atrophy correction,aged 24 to 49 years old,with an average age of 28.42 years,of which 24 cases were corrected in the middle and lower parts of the affected side.There were 4 cases with filling correction on the upper,middle and lower parts of the affected side.The surgical method uses the special technology of our department to fill the recessed area with SVF-gel,with 10%overfilling as the filling volume.Results Nineteen patients were satisfied with the results of the operation 3 months after the initial operation,and the initial operation satisfaction reached 67.86%;7 cases received secondary fillings and 2 received three fillings.Complications:All patients had a single filling volume of more than 20ml,and there were no complications such as infection,liquefaction,and nodules.In the long-term follow-up,the filling area was in good result.Conclusion The effect of autologous SVF-gel filling in improving hemi-facial atrophy is better than that of traditional autologous fat.The postoperative recovery is quicker and the retention rate is significantly improved.It can also be frozen for secondary filling to reduce the burden on patients.It is worthy of clinical application.
