基于络病理论的清胃通络方对于慢性糜烂性胃炎(CEG)伴癌前期病变(PLGC)临床疗效的研究

摘　　要：目的观察清胃通络方治疗慢性糜烂性胃炎伴癌前病变的临床疗效。方法将80例中医辨证为湿热阻络型的CEG伴PLGC患者随机分为两组,治疗组44例,对照组36例。对照组给予胃复春片治疗,治疗组给予清胃通络方治疗,疗程6个月。比较两组患者治疗前后中医症状积分、内镜疗效、病理改善情况(萎缩、肠化、异型增生)、HP清除率以及G-17、PGI、PGⅡ和PGR情况。结果中药治疗组在改善CEG伴PLGC患者临床症状、内镜疗效、病理疗效、HP清除率方面较对照组有显著性差异(P<0.05);两组PGI、PGR水平均较治疗前增加(P<0.05),且治疗组优于对照组(P<0.05),而G-17亦较治疗前有所改善。结论基于中医络病理论的清胃通络方通过清胃化湿、祛瘀通络治疗CEG伴PLGC,临床总体疗效较好,可明显改善患者的临床症状,逆转患者胃黏膜萎缩、肠化及异型增生方面具有显著的疗效。Objective To observe the clinical effect of Qingwei Tongluo Recipe on chronic erosivegastritis with precancerous lesions.Methods 80 cases of CEG with PLGC with damp heat blocking collaterals were randomly divided into two groups,44 cases in the treatment group and 36 cases in the control group.The control group was treated with Weifuchun tablets,and the treatment group was treated with Qingwei Tongluo recipe for 6 months.The TCM symptom score,endoscopic efficacy,pathological improvement(atrophy,intestinal metaplasia and dysplasia),HP clearance rate,G-17,PGI,PGⅡand PGR were compared between the two groups before and after treatment.Results There were significant differences in clinical symptoms,endoscopic efficacy,pathological efficacy and HP clearance between the traditional Chinese medicine treatment group and the control group(P<0.05);The levels of PGI and PGR in the two groups were higher than those before treatment(P<0.05),and the treatment group was better than that in the control group(P<0.05),and G-17 was also improved.Conclusion Qingwei Tongluo recipe based on the collateral disease theory of traditional Chinese medicine has a good overall clinical effect in the treatment of CEG with PLGC by clearing stomach,removing dampness,removing blood stasis and dredging collaterals.It can significantly improve the clinical symptoms and reverse the atrophy,intestinal metaplasia and dysplasia of gastric mucosa.
