基于数据挖掘探讨针刺治疗脑卒中后认知障碍的选穴规律

摘　　要：目的探析脑卒中后认知障碍针刺疗法的选穴规律。方法以“针刺”或“针灸”或“针”和“认知障碍”为主题词,检索中国期刊全文数据库(CNKI)、万方数据库、维普数据库(VIP)、PubMed、Web of Science和EMbase等数据库中针刺治疗脑卒中后认知障碍的临床研究文献,建立Excel腧穴数据库,运用聚类分析和关联规则分析方法,探析临床针灸处方用穴的特点和规律。结果①针刺治疗脑卒中后认知障碍以督脉、足少阳胆经、足阳明胃经、足太阴脾经的腧穴及经外奇穴为主,常用腧穴主要以头颈部分布为主,腧穴以特定穴为主,主要包括交会穴、原穴、输穴、络穴、合穴等。②关联规则分析显示腧穴间支持度较高的关联群是百会-四神聪、百会-神庭、百会-风池,通过聚类分析得出5个有效聚类群:水沟-神门、本神-大椎-肾俞-丰隆-太溪、膻中-中脘-血海、合谷-曲池-太冲、三阴交-内关。结论现代针刺治疗脑卒中后认知障碍尤其重视百会-四神聪、百会-神庭、百会-风池与其他腧穴配伍,体现了“治病求本”的治疗原则,“经脉所过,主治所及”“病位所在,主治所在”的选穴原则,远近配穴、上下配穴的配穴方法,重用特定穴。Objective To explore the rules of acupoint selection in acupuncture therapy for post-stroke cognitive impairment.Methods With"acupuncture"and"cognitive impairment"as the subject words,the clinical literatures on acupuncture treatment of post-stroke cognitive impairment in CNKI,VIP,Wanfang,PubMed,Web of Science,EMbase databases were collected,and excel acupoint database was established.Cluster analysis and association rule analysis were used to analyze the characteristics and rules of acupoint selection in acupuncture therapy for post-stroke cognitive impairment.Results Acupuncture treatment of post-stroke cognitive impairment mainly includes Du meridian,foot Taiyang bladder channel,foot Shaoyang gallbladder channel,and foot Yangming stomach meridian,and extra meridian points.The common acupoints were mainly distributed in the head and neck,and the acupoints were mainly specific points,including intersection points,original points,Shu points,collateral points,and combined points.Association rule analysis showed that Baihui Sishencong,Baihui Fengchi,and Baihui shenting with the highest correlation.Through cluster analysis,five effective clusters were obtained:Shuigou Shenmen,Benshen Dazhui Shenshu Fenglong Taixi,Tanzhong Zhongwan Xuehai,Hegu Quchi Taichong,Sanyinjiao Neiguan.Conclusion For modern acupuncture in the treatment of post-stroke cognitive impairment,special attention were paid to the compatibility of Baihui Sishencong,Baihui Fengchi,and Baihui Shenting with other acupoints,which reflects the treatment principle of"treating the disease and seeking the root",the selection principles of"meridians,main treatment points"and"disease location,main treatment location",the acupoint matching methods by far and near points,upper and lower points,and reuse of specific points.
