头痛宁胶囊治疗痰瘀阻络型紧张性头痛的临床研究

摘　　要：目的观察头痛宁胶囊治疗痰瘀阻络型紧张性头痛(TTH)的临床疗效及对病人脑血流、血浆降钙素基因相关肽(CGRP)和5-羟色胺(5-HT)水平的影响。方法选取成都市第三人民医院2016年8月—2019年10月收治的132例痰瘀阻络型TTH病人为研究对象,随机分为观察组(67例)和对照组(65例),对照组给予氟哌噻吨美利曲辛片治疗,观察组在对照组基础上加用头痛宁胶囊治疗,观察两组临床疗效、症状缓解情况、脑血流速度、血液生化指标[CGRP、血浆内皮素(ET-1)和5-HT]水平及不良反应。结果治疗4周后,观察组临床总有效率为92.54%,对照组临床总有效率为80.00%,两组比较差异有统计学意义(P<0.05)。观察组治疗后视觉模拟评分法(VAS)评分低于对照组,头痛发作次数少于对照组,头痛持续时间较对照组缩短,差异均有统计学意义(P<0.05);观察组治疗后血浆CGRP、ET-1水平低于对照组(P<0.05),5-HT水平高于对照组(P<0.05);观察组治疗后大脑前动脉(ACA)、大脑中动脉(MCA)、大脑后动脉(PCA)的最大血流速度(Vs)及平均血流速度(Vm)较对照组增加明显(P<0.05)。两组不良反应发生率比较差异无统计学意义(P<0.05)。结论头痛宁胶囊可明显改善痰瘀阻络型TTH病人的头痛症状,加快脑部血液流通,促进血管舒张。Objective To explore the effects of Toutongning capsule on tension-type headache of phlegm-stasis blocking collateral type and its influence on cerebral blood flow and the levels of plasma calcitonin gene-related peptide(CGRP)and 5-hydroxytryptamine(5-HT).Methods A total of 132 patients with tension-type headache of phlegm-stasis blocking collateral type were selected,and were divided into observation group(n=67)and control group(n=65)according to the randomized grouping method.The patients in control group were given flupentixol and melitracen tablets,and the patients in observation group were treated with Toutongning capsule on the basis of control group.The clinical efficacy,symptom relief,cerebral blood flow velocity,levels of blood biochemical indexes[CGRP,plasma endothelin-1(ET-1),and 5-HT],and adverse reactions were observed in two groups after 4 weeks of treatment.Results After 4 weeks of treatment,the clinical effective rate in observation group was higher than that of control group headache in(92.54%and 80.00%,P<0.05).Compared with control group,the visual analogue scale(VAS)score,attack frequency and duration of observation group decreased significantly(P<0.05).The plasma levels of CGRP and ET-1 in observation group were lower than those in control group(P<0.05),and the level of 5-HT in observation group was higher than that in control group(P<0.05).The maximum blood flow velocities(Vs)and mean blood flow velocitie(Vm)of middle cerebral artery(MCA)and posterior cerebral artery(PCA)in observation group increased significantly Compared with those in control group(P<0.05).After 4 weeks of treatment,there were no significant differences in the adverse drug reactions between two groups(P>0.05).Conclusion Toutongning capsule can significantly improve the headache symptoms,speed up blood circulation in the brain,and promote vasodilation of patients with tension-type headache of phlegm-stasis blocking collateral type.
